South Bristol Rehabilitation units are located within South Bristol Community Hospital and has built up a reputation for delivering excellent patient centred care and enthusiastic teamwork.
As a Healthcare Support worker you will assist with the direct care of patients delegated to and supervised by a registered nurse.
Your role will provide support to the nursing team, collaboratively with the multidisciplinary team and contribute positively to the delivery of high-quality, compassionate care centred around the core principles of recovery and stroke rehabilitation and the patients wishes.
The service supports patients to recover and regain their independence following admission to an acute hospital and the team consists of registered nurses, therapists, advanced clinical practitioners, General Practitioners and a range on unregistered support workers.

Main duties of the job
To assist registered nurses and healthcare professionals in the delivery of individualised holistic care which is delivered with compassion, dignity and respect.
Work with the multidisciplinary team and contribute to the prevention of harm by participating risk assessments such as nutrition, pressure area care, continence and falls.
To assist patient with their care and nutritional support and needs.
To safely and accurately record physiological observations such as blood pressure, pulse, respirations and temperatures and escalate and report to the registered nurse accountable for the patients care.
To report back any relevant care about the patient to the registered nurse.
Assist patients to mobilise safely using equipment and techniques prescribed and recorded in their plan of care.
Promote a culture of ambulatory care by supporting patients to be as independent as possible with washing, dressing in own clothes and encouraging mobility from admission.
Assist in the provision of prescribed programmes of care e.g rehabilitation and therapy goals under supervision and guidance.
Support patient centred rehabilitation to facilitate safe and timely discharges and which reflects individual choice and maximising independence.

Essential criteria- NVQ Level 3 in Health and Social Care (Adults) or equivalent experience
Experience, skills and knowledge
Essential criteria- Significant experience of direct patient care in a health or social care setting
- Ability to plan and prioritise effectively and efficiently individually and in a team environment
- Able to communicate clearly and effectively with a variety of service users with a range of communication needs and/ or cultural backgrounds.
- Able to carry out the core clinical competencies required of the post e.g. wound care, observations
- Able to record succinct and accurate information on the patient record both in writing and electronically
- Experience in community and acute settings
