Excellent communication skills are essential for the role. Previous experience of Community Mental Health work is desirable.
Blackburn with Darwen Community Mental Health Team are seeking to appoint a Band 6 Community Mental Health Practitioner with a view to consider appointing a band 5 progressing to a band 6.
This post provides the opportunity for a forward thinking and experienced individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care.
With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be an innovative thinker and be able to form effective relationships. Over a period of 12 months, you will be expected to progress to a Band 6 practitioner.
You must be a registered mental health practitioner, with excellent leadership and clinical skills.
You must have some post qualification experience in clinical work with service users experiencing a range of mental health problems.
You need to be able to work as part of a multidisciplinary team and have experience of planning, implementing and reviewing care needs.You will also need to have a working knowledge of the care programme approach (CPA), Dialog+, risk assessment and management, and the ability to explore issues relating to the impact of health and social needs on mental health and well-being.
You will be a mental health (MH) practitioner working within a multidisciplinary team and carry out a diverse range of duties.
As a Band 5 MH practitioner within the team you will be regularly supported and supervised by a nominated Band 6 Nurse and the clinical lead/team leader, you will be encouraged to take personal responsibility for clinical supervision and your continued professional development plan.
You will be required to undertake the administration of medications and safe storage of same.
You will actively promote teamwork, respect, integrity, accountability, excellence and compassion within the clinical team in accordance with the Trust values and Code of Conduct. You are required to maintain links with your professional body and provide evidence for the revalidation process.
You will provide support to the nurse leadership within the multidisciplinary team and ensure the effective assessment, formulation, planning and monitoring of care given to service users.
You will exercise professional accountability and responsibility using skills, knowledge and expertise in changing environments, across clinical boundaries and in unfamiliar situations as outlined within the NMC Code of Professional Conduct.
You may be required to take on tasks that require you to operate electronic staffing record systems such as Appraisals/supervision.
You will maintain a positive and open attitude towards organisational change and service development.

Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.
