An exciting opportunity has arisen to join our multidisciplinary community team as a Diabetes Nurse within the Doncaster Community Diabetes Specialist Nursing Team.
The Diabetes specialist Nursing service is part of RDaSH who is a forward-thinking organisation and supports development and innovation at all levels to enhance the quality of patient care and service development.
The aim of the service is to promote self-care of their condition; to avoid unnecessary hospital admissions and support timely hospital discharges to maximise individual potential for independent living within a person’s usual place of residence.
The successful candidate will possess excellent communication, organisational and problem-solving skills and will be able to manage their own workload effectively under the supervision of the Diabetes Specialist Nurses.
Post holders must be able to demonstrate competence and confidence in a wide range of relevant clinical skills and knowledge.
Main duties of the job
The diabetes nurse is a qualified, experienced nurse with specific training in the condition of diabetes mellitus and its management.
Under the supervision/direction of the Diabetes Nurse Specialist, the diabetes nurse will work across settings, to deliver specialist clinical diabetes services to adults with diabetes.
Provide specialist interdisciplinary education and training to other staff and students.
Work with the team to influence the development of the service both locally and nationally.
Participate in clinical audits in own area.
Maintain high standards of professional care.

Qualifications
Essential criteria- First level nurse
- Diabetes experience
- Qualification or equivalent experience in teaching and assessing
- Community experience
- Evidence of Degree level of study
