Job overview
The North West Kidney Network (NWKN) has a workstream dedicated to chronic kidney disease (CKD) in Greater Manchester. In March 2024 they applied for transformational funds from Specialised Commissioning North West with agreement from Greater Manchester (GM) Integrated care board (ICB) team to undertake a CKD Pilot in the Greater Manchester area.
The post holder will work alongside CKD lead and QI Manager for the Network. Establish a Network of collaboration amongst GP practices within Oldham and Tameside.
Main duties of the job
The post holder will work alongside CKD lead and QI Manager for the Network and establish a Network of collaboration amongst GP practices within Oldham and Tameside. They will standardise documentation and answer emails promptly. In addition:
- Arrange appropriate IT access where appropriate.
- Arrange clinic room space where appropriate.
- Arrange regional webinar (1 per locality)
- Arrange webinars between primary care and nephrologist.
- Liaison between QI manager and practices RE CDRC visits
- Manage and attend virtual MDTs.
- Record and report engagement and outcomes
