We are seeking a highly motivated and dedicated Clinical Nurse Practitioner to join our Shrewsbury based Intensive Support Team in Shropshire. This is an exciting opportunity for an experienced RNLD or suitably experienced nurse.

The role requires a professional who is enthusiastic, dynamic and able to demonstrate excellent inter-personal communication skills and a determined commitment to person-centred, multi-disciplinary working to achieve quality of life outcomes.

Main duties of the job

The aim of the Intensive Support Team (IST) is to provide specialist and intensive multi-disciplinary support in the community for people who have learning disabilities; there is a core functionality role within the team in addition to clinicians’ individual specialist roles.

Experience of working with people with a learning disability is essential. You will also preferably have experience of working with people who have complex needs, for example those associated with mental and physical ill health, autism and behaviours that challenge.

The ideal candidate will be somebody who is passionate about service delivery to and for people with learning disabilities, can work creatively and innovatively to secure good outcomes, and embrace new ways of working within a Positive Behaviour Support Framework.

Detailed job description and main responsibilities

You will work closely with your IST colleagues to support a defined, shared caseload of adults who may be at risk of placement breakdown, including individuals in transition from children’s services, specialist inpatient services or moving between community placements.

You will be using flexible, creative, multidisciplinary approaches and make a significant contribution to the continued development of high quality local services in Shropshire for people who have a learning disability. Y
our role will include providing person-centred assessment, to advise and implement clinical interventions based on an MDT formulation.
On occasion this may involve planned out of hours work, with access to on-call support and supervision, if the needs of an individual require it. You will work collaboratively alongside the Specialist Community Learning Disability Teams, in-patient services and statutory / independent sector provider services.

You can expect specialist clinical training and updates, regular support and supervision and professional leadership.

Person specification

Qualifications & Training

Essential criteria
  • Professional qualification – RNLD or suitably experienced nurse
  • Current professional registration with the NMC
  • Evidence of continuing professional development and maintenance of portfolio
Desirable criteria
  • Qualification in supervision or mentoring.
  • Evidence of relevant training or currently undertaking e.g. Positive Behaviour Support, Applied Behaviour Analysis

Experience

Essential criteria
  • Post registration experience in working with people with Learning Disabilities with complex health care needs e.g. Behaviour that Challenges, Mental Health needs
  • Experience of working within multi-disciplinary teams
  • Experience of managing and prioritising a caseload
Desirable criteria
  • Experience of providing clinical supervision for others
  • Experience of MDT clinical formulation, to prescribe and implement person centred recommendations

Skills, Knowledge & Abilities

Essential criteria
  • Knowledge of current trends and legislation impacting on services provided to people with a learning disability
Desirable criteria
  • Knowledge of the Transforming Care agenda and the DSR, C(E)TR and LAEP processes
Visa eligibility notice: The UK Home Office has imposed an emergency brake on specific visa routes.
  • Skilled Worker visas cannot currently be issued to Afghan nationals.
  • Study visas cannot currently be issued to nationals of Afghanistan, Cameroon, Myanmar or Sudan.
