Job overview

Stockport Foundation Trust is currently offering an exciting opportunity for an Advanced Clinical Practitioner to join our multidisciplinary Rheumatology service.
  • The ACP will autonomously provide highly specialised, evidence-based care within the rheumatology team and will demonstrate advanced clinical knowledge and skills in assessment, management, and treatment across a range of rheumatology presentations
  • The ACP will work collaboratively with all other MDT rheumatology members to ensure a high-quality service. This will include working in different outpatient settings, the daycase units as well as potentially review of patients on wards
  • The ACP will respond to patients calls through the rheumatology advice line service
  • The ACP will encompass the four pillars of clinical practice, leadership and management, education, and research, with demonstration of core capabilities and area specific clinical competence.

Main duties of the job

The post holder will be responsible for providing advanced clinical care and expertise in care delivery including judgement, discretion and decision making to patients as per individual scope of practice and through the advanced clinical competency framework.

Detailed job description and main responsibilities

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trust’s policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.

Person specification

Education

Essential criteria
  • Professional Registration
  • Non- Medical prescribing course (in eligible professions)
  • MSc in clinical practice
Desirable criteria
  • Teaching or relevant experience qualification

Knowledge

Essential criteria
  • Advanced clinical practice skills
  • Evidence of research and audit
  • Evidence of implementing change
  • Evidence of dissemination of knowledge in wider healthcare organisations

Experience

Essential criteria
  • Post registration experience at a senior level (band 6 or above) in community or a relevant speciality area
  • Evidence of on-going CPD
  • Evidence of teaching in a variety of environments
  • Ability to work independently.
  • Experience of using multidisciplinary approach to patient care

  • Strong leadership skills
  • Excellent communication skills and able to communicate with MDT
  • Able to problem solve and make decisions
  • Ability to prioritise tasks, actions and interventions

  • Research and audit awareness
  • Able to work as part of a team and autonomously
