Main duties of the job

We are looking for a Band 4 Senior Therapy Assistant to join our Community Neuro Rehab Team.

We provide short-term rehabilitation to enable patients, where possible, to remain in the community rather than be admitted to hospital or long-term care, and to live as independently as possible.

We work within patients’ homes, rehabilitation groups, Community

Hospitals and other care settings, to ensure effective discharge planning and a seamless rehabilitation pathway. We provide holistic assessment of patients’ needs and help patients achieve maximum independence in daily living activities and improved well-being.

Your role will be absolutely key in the support of our patients. You will help with the undertaking of treatment plans and use your clinical judgement to keep people as safe as possible. Full duties can be found in the job description.

Please note due to the community based nature, you will need a driving licence and a vehicle.

Additional Working Pattern: 7.5 hours each day Wednesday to Friday

Person specification

Qualifications

Essential criteria
  • Functional Skills at level 2 (literacy and numeracy) / GCSE Grade A-C or equivalent in English and Maths
Desirable criteria
  • Evidence of Foundation Degree Level study (level 5) or mentorship/coaching qualification

Experience

Essential criteria
  • Minimum of 2 years' experience as a Senior Healthcare Worker
Desirable criteria
  • Experience working within community environment and/or prior work with patients with physical health conditions as B3 / B4

Knowledge

Essential criteria
  • Understands the role and remit of a Band 4
  • Awareness of professional issues and recent developments in the NHS and Integrated Care Board including Clinical Governance, NICE, Essence of Care

Skills & abilities

Essential criteria
  • Excellent communication and interpersonal skills.(verbal and written)
  • Experience of working with IT packages (such as Microsoft Office, e-record systems)
  • Valid driving licence in the UK and a vehicle

  • Adaptable and flexible to service demands
  • Ability to motivate and organise others to ensure best practice
