Job overview
The Community Stroke Rehab Team provides stroke specialist rehabilitation for Enfield residents aged 18+ with an NCL GP. Care is provided in people’s home and local communities dependent on their functional goals. It is a friendly multidisciplinary team of occupational therapists, physiotherapists, speech and language therapists and neuropsychology.We have generic rehabilitation assistants to support people to carry out treatment plans set by the therapists. We work closely with adult community therapies who are also located within St Michaels.
We offer in-service team training, effective supervision and external training opportunities through the Trust.We are offering an exciting opportunity for a highly motivated Speech and Language Therapist who wishes to develop a their stroke specialist skills within community services.
The services are located near Enfield Town, with easy commuter links from central London/Hertfordshire and proximity to M25.
A car driver is essential for this role.
Main duties of the job
To undertake a comprehensive assessment of patients presenting with acquired communication and swallowing difficulties, including those with diverse or complex presentations/multi pathologies following stroke: using a high level of clinical reasoning and assessment techniques to provide an accurate clinical diagnosis.
Where the service is available, to identify and refer patients for video fluoroscopy and other objective swallowing assessment techniques.
To utilise an extensive knowledge of evidence-based practice and treatment options to select and deliver a specialised speech & language therapy programme to individual patients and plan their successful discharge.
To take responsibility for a delegated caseload, ensuring that the MDT is fully involved as required
To ensure that patients and carers are involved in the planning of their care and rehabilitation, using negotiation skills as appropriate
To assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
To provide education and training relevant to communication and dysphagia rehabilitation to MDT colleagues, patients , carers and families, as well as support community care workers with supporting these needs.

Education and qualifications
Essential criteria- RCSLT Recognised Diploma/Degree in SLT
- RCSLT Recognised postgraduate dysphagia qualification
- Evidence of up to date RCSLT CPD portfolio
- RCSLT attendance at relevant post-graduate courses
Skills and abilities
Essential criteria- Ability to prioritise, organise and work in a busy environment
- Competent IT skills- evidence of use of basic microsoft packages
- Able to manage complex situations e.g. dealing with aggressive and /or emotional patients or carers effectively and appropriately
- Ability to work effectively wihtin a team environment
- Good communication skills, able to present information (written and oral) in a clear and logical manner)
- Experience of organising internal or external training course
Experience
Essential criteria- Minimum 18 month- 2 years post graduate clinical experience
- Extensive post-graduate dysphagia qualification experience
- Clinical experience working in NHS
- Experience working with patients with acquired neurological conditions
- Experience working in outpatients and/ or community MDT settings
- Participation in project/ audit activities
- Experience of assessment and implementation of low and high-tech Alternative and Augmentative Communication (AAC)
Personal Qualities/ Knowledge/ Awareness/ Understanding
Essential criteria- Well developed knowledge of evidence based assessment and therapeutic approaches for patients with aquired medical condition
- A good working knowledge of the NHS systems of care and awareness of current NHS policies and issues which impact on the SLT profession and delivery of care.
- Awareness of own knowledge and developmental needs and learning style
- Extensive knowledge base underpinning broad SLT skills.
