Pharmacy Technician

Job Summary: The Enhanced Care in Care Homes Pharmacy Technician will support the Darlington PCN Pharmacy Hub by working with the pharmacy team and GP practices to deliver a programme of medicines optimisation and reconciliation from hospital discharge and other secondary care communications.
This will enable safe discharge of patients from hospital to home, ensuring medication changes are safe and timely managed with patient counselling where appropriate. The post holder will work within their own competency with support from a senior technician and pharmacist.
The post holder will support the pharmacy team with specific quality improvement projects in relation to medicines for example opioid reduction management. Initial key responsibilities: Simple medication changes from ANP or GP or Practice pharmacists- care home will need a medication change form completing and emailing to them.
BP reviews- if unable to undertake these can be passed to care home GP or hub pharmacist. MDT follow up- training can be given. Visits to Care homes to ensure medication compliance. Discharge review follow up. Care home medication requests for missing monthly items.
Care home requests for interim items or when required medication. Care home advice CPN medication changes- usually letter attached to email, processed in same way as if coming into hub workflow. Additional responsibilities commensurate with the role Aims/Goals of the Role Reduced GP workload Reduced medication costs by formulary prescribing cost-effective alternatives.
