An exciting opportunity has presented itself for a permanent Band 2 Health Care Support Worker to work on the Discharge Waiting Area.

The successful candidate will be instrumental in maintaining patient flow across the Trust by facilitating smooth and efficient patient discharge.

Working with the team, the post holder will ensure safe and timely transfer of discharges to the unit and more importantly, a positive patient experience through the delivery of high standard of patient care.

You will be supported by, and will work closely with DWA In Charge and DWA Coordinator.

The Discharge Waiting Area is now 24/7.

Facilitate transfer of patients from the base ward to DWA

Support the Registered Nurse/AP in the day to day running of the Discharge Waiting area.

Ensure that the patients are receiving optimal care whilst under the care of DWA

Key Tasks & Responsibilities:

To assist in the delivery of a high standard of patient care in all activities of daily living, as directed in the individual plan of care needs, observing and reporting specific changes to senior staff and documenting care given.
  • Support the Registered Nurse/AP in the day to day running of the Discharge Waiting area.
  • Ensure that the patients and relative have a positive experience.
  • Ensure the dietary and hydration needs of all patients in the Discharge Waiting Area are met.
  • Ensure all patients requiring assistance with meeting their activities of daily living have access to help
  • Assist with nursing care as required
  • Assist with the transfer of patients to Discharge Waiting Area
  • Inform nurse/AP in charge if patients voice any concern about their discharge
  • Assist in collecting TTOs from the Pharmacy
  • Undertake basic cleaning and decontamination of communal equipment between patient use and/or storage.

Person specification

Education & Qualification

Essential criteria
  • Good general education; GCSE passes (or equivalent) in English and Maths
  • Care Certificate or commitment to undertaking and achieving the Care Certificate within initial twelve weeks
  • ICT basic skills
Desirable criteria
  • Vocational qualification at Level 2 in Health, Health and Social Care or sector related subject area

Experience & Knowledge

Essential criteria
  • Experience of working / communicating with the general public
  • Communication with a range of different people including service users, carers and members of the multidisciplinary team
  • Working in an effective team
Desirable criteria
  • Experience of working in a healthcare team environment

Skills & Abilities

Essential criteria
  • Communicates clearly in writing, verbally and using the telephone
  • Demonstrates awareness of the 6C’s, Trust values and Patients First Service Standards
  • Able to understand the importance of how to apply Person Centred Values in the care environment: privacy, dignity, respect, active participation, equality, diversity, confidentiality
  • Demonstrates an awareness of the importance of keeping the clinical environment safe and clean
  • Able to use information technology devices
Desirable criteria
  • Knowledge of the Data Protection Act
  • Understanding of risk management and health and safety issues, including safe manual handling
