Job overview
An exciting opportunity has arisen for a Band 5 Community nurse working within the Northwest Integrated Neighbourhood Team based at Hartismere Hospital in Eye. This is a 12-month fixed term/secondment opportunity. Please note: This could be a Band 6 position depending on relevant experience.This will be assessed at interview
The team consists of district & community nurses, assistant practitioners, generic workers, occupational therapists, physiotherapists, and social care workers. The team is led by a Nursing Clinical Lead, integrated with a Therapy Clinical Lead and overall managed by an INT manager.You will be required to work across the Northwest INT, and the wider community services to meet service requirements
The team is proud of their holistic approach to care. The team continues to change to support developments in NHS which includes close working relationships with the local statutory, non-statutory and voluntary services
You will be actively supported by regular supervision and individual annual appraisal. You will be expected to work autonomously and on behalf of the patients served by the local area. You will have access to skills development training relevant to the post
You will be required to work shifts covering the operational hours of 08:00 to 18:00 over 7 days
Due to being a community post applicants must hold a UK driving licence and have access to transport from the start of their employment
Main duties of the job
Your duties would include wound care, leg ulcer management, pressure area care, palliative care, venepuncture, attending GP multi-disciplinary team meetings, PICC line care, male, female suprapubic catheterisation, ordering equipment, support and mentoring of students and involvement in link roles such as tissue viability.
We offer a variety of training to up skill and maintain your clinical skills.
The team will provide support through in-service training and professional forums, mandatory training, 1:1 and personal development meetings as well as clinical rolling skills training via the practice development facilitators. We facilitate students on training and have close working relationships with neighbouring community teams in Health and Social Care.

Person specification
Qualifications
Essential criteria- RN Qualification (Part 1 of register) or equivalent
- Current NMC registration
- Leg Ulcer Management
- IV Administration / cannulation
- ENB 998/mentorship preparation or equivalent
- Mentorship qualification is essential for the post, but not essential at interview
- Previous experience working in the community.
Skills, Abilities & Knowledge
Essential criteria- Effective observational and reporting skills
- IT and standard keyboard skills
- Leadership Skills
- Able to manoeuvre patients using handling aids
- Able to manoeuvre patients using handling aid
- Clinical reasoning skills
- Car owner/driver/access to a motor vehicle (role will involve travel to other sites that cannot be easily accessed by public transport means)
