The Wokingham Older Adult Mental Health Service is looking for an enthusiastic, caring and highly motivated Mental Health Practitioner with a relevant and current professional qualification (Social Worker, Mental Health Nurse or Occupational Therapist) to join our friendly and innovative Memory Clinic.

Your base would be Wokingham Community Hospital where we have access to free parking facilities, staff room and a friendly and welcoming team. We work flexibly to meet service needs both from the office and remotely.

Wokingham Memory Clinic is proud to be an accredited Memory Clinic by the Royal College of Psychiatrists Memory Services National Accreditation Programme (MSNAP). Combined with being an ‘Outstanding’, well-led, organisation, and having an ‘Outstanding’ Older Adults Community Mental Health Team in our latest CQC inspection.

The post holder would therefore be expected to work towards helping the service maintain these accolades of high standards.

Main duties of the job
  • The post holder will undertake comprehensive assessments in a range of settings including the patient’s own home
  • Participate in planning and facilitating group work including cognitive stimulation therapy, as well as group work within the wider service where required.
  • To provide the appropriate clinical follow up
  • To identify and select from a range of health, social and voluntary agencies which could assist and improve the care of individuals and their families.
  • To be jointly responsible for the Memory Clinic caseload working collaboratively with other Memory Clinic practitioners.
  • To monitor and evaluate care plans, making adjustments as appropriate

Detailed job description and main responsibilities

The “must haves” for this role:

  • Hold the relevant and current professional qualification (Social Worker, Mental Health Nurse or Occupational Therapist)
  • To have excellent problem-solving abilities, communications skills and a person-centred approach
  • A passion for working with older adults and those living with dementia
  • Group work experience
  • Valid UK licence with the ability to travel in a timely manner between work bases and community settings, including transporting patients where required. (public transport would not be suitable)

Education Qualifications Training

Essential criteria
  • RMN or equivalent Professional Qualification (OT, SW)
Desirable criteria
  • RGN (i.e. dual qualified)
  • Mentorship or equivalent
  • Diploma in Dementia Studies or willing to undertake
  • Completion of post-graduate courses related to specialism e.g. Advanced Dementia Care, Cognitive Behaviour Therapy and Communicating & Caregiving in Dementia etc

Continuous Professional Development

  • Evidence of Statutory & Mandatory Courses

Previous Experience
