The undertaking of a Physical Health assessment for patients diagnosed with a Severe Mental Illness (SMI), either in clinic or as part of a home visit. The role necessitates accurate, comprehensive and contemporaneous documentation in patient records and appropriate communication with other health care professionals including the patient's GP Supporting the patient with health interventions that may be required as a result of your findings.
Attending training as required and keeping up to date with developments in field of care as appropriate. Working as part of a team, adhering to NSFT values. Please refer to the Job Descriptions for further details
