We are looking to recruit an enthusiastic and motivated Pharmacist to undertake the role of Medication Safety Officer and be responsible for advancing the Antimicrobial Stewardship agenda within the Trust.

Our pharmacists play an integral role as members of the multi-disciplinary team and this post will suit a pharmacist wishing to join an inclusive and friendly working environment to improve medicines safety and antimicrobial stewardship across the organisation.

You will also work as part of the wider clinical pharmacy team providing advanced clinical pharmacy advice to a specific clinical area.

The ideal candidate will have a commitment to teamwork, a sense of

accountability, plus an ability to work under pressure.

We will provide the necessary support to ensure your development within your role.

The pharmacy team has various bases across the county and the successful candidate will be based at one of them with some flexibility for hybrid working, if agreed. It is essential to be able to travel to various sites across the Trust.

Main duties of the job

To undertake the role of Medication Safety Officer for the Trust including representing the Trust on national, regional and system level medication safety networks. This trust wide role leads on all aspects of medication safety within the Trust, working closely with a wide range of multidisciplinary clinical staff and teams and the patient safety team.

This role is crucial for improving medication safety and learning from medication incidents within the Trust. This includes promoting the safe use of medicines, improving the quality of reporting, serving as an essential link for the implementation of local and national medication safety initiatives and alerts.

To lead on Antimicrobial Stewardship for the Trust, working closely with the Infection Control Team and a wide range of multidisciplinary clinical staff and teams.

To undertake clinical pharmacy research and audit to influence practice within the Trust and inform the development of protocols and guidelines associated with the safe use of medicines in mental health, learning disability and community healthcare services.

To be a member of the Clinical Pharmacy team. Provide an Advanced Clinical Pharmacy service to a specific clinical area as required, prescribing medicines within scope of practice if an Independent Prescriber.

  • Be the Trust Named Medication Safety Officer.
  • Function as the organisational link with NHS England and the Medicines Healthcare and Regulatory Agency to receive essential communication and escalate concerns about safe use of medicines.
  • Be a member of the National Medication Safety Network, ensuring relevant national initiatives and learning are implemented within the Trust
  • Represent the Trust at regional and system level Medication Safety networks
  • Be an active member of the Trust Medicines Governance Group. This acts as the Trust medicines safety group.
  • Update the Trust Medication Safety programme, ensuring it develops in line with national, regional and local initiatives as well as being informed by Trust safety priorities and learning from incidents.
  • Lead on the implementation of medicines related National Patient Safety alerts, collaborating with the wider clinical teams
  • Encourage the reporting of medicines incidents by all staff
  • Improve the quality of medication error reporting including review and reclassification of individual incidents for data quality purposes as required
  • Produce the Trust Quarterly Medication Safety report, demonstrating oversight of medicines incidents, identification of trust wide trends and themes and learning from them.
  • Work in partnership with other professionals and system partners to reduce the number of moderate and severe harm medication related event throughout the Trust and ICS. Identifying and implementing new ways of working as required.
  • Lead on the development and update of Trust medication safety related policies
  • Support the development of Trust electronic prescribing and medicines administration in relation to medication safety
  • Work closely with the Patient Safety Team in the implementation of Patient Safety Incident Response Framework, participating in learning activities and any investigations as needed with regards to medicines
  • Undertake training to be able to undertake Structured Judgement Reviews
  • Support the Accountable Officer for Controlled Drugs in relation to controlled drug incident reporting, investigation and producing reports

Person specification

Education / Qualifications

Essential criteria
  • Registration with GPhC.
  • Post-graduate clinical pharmacy qualification or equivalent experience
Desirable criteria
  • Independent Prescriber or working towards an independent prescribing qualification
  • Member of Royal Pharmaceutical Society

Experience

Essential criteria
  • At least 3 years postgraduate employment as a pharmacist
  • Experience in two or more of hospital pharmacy, mental health, medicines safety, clinical pharmacy role, research, clinical trials and/or audit, medicines protocol/guideline development and implementation, supervisory role
Desirable criteria
  • Management of staff
