Are you passionate about empowering young people and making a positive impact? We’re seeking a Young Person Worker to join our team, providing county-wide, holistic support to individuals aged 10 to 25 who are affected by substance misuse, including issues stemming from familial substance misuse.

This post will be specifically to work alongside the YOS team

What You'll Do:

  • Deliver tailored, young person-centered interventions to help reduce risks, increase resilience, and achieve positive outcomes.
  • Manage a caseload of young people referred from diverse pathways, including health, social care, education, and criminal justice.
  • Work flexibly in the community and multi-agency settings on an outreach basis to meet the needs of young people and their families.
  • Facilitate drop-in and group sessions to engage harder-to-reach young people.
  • Use a solution-focused approach to manage crises with resilience and compassion.
  • Promote recovery and positive outcomes for young people and their families

Main duties of the job
  • To promote a strength- based approach with young people to empower change, to reduce risk and build resilience.
  • To support and build relationships with 10- to 25-year-olds, and their families, who have a variety of needs related to substance misuse, either their own substance use or somebody else’s.
  • Hold a caseload of young people and complete all recording and case management administration for your caseload accurately and promptly, adhering to GDPR and Data protection policies.
  • Develop co-produced, outcome focused support plans that reflect each participant’s personal strengths and goals. Support plans will be holistic, covering all aspects of the young person’s wellbeing. This will include the engagement of parent/carers, and family members where appropriate.
  • To work as part of a multi-agency team delivering interventions to safeguard children and vulnerable adults at risk and to reduce drug and alcohol related harm to young people, families, and communities.
  • To provide information and advice to colleagues regarding the risk and protective factors surrounding young people with complex needs.

  • To engage young people in a variety of interventions aimed at increasing choice and control in their lives, using a young person centred, assertive outreach approach.
  • Providing screening, strength-based assessment, psychosocial interventions, integrated and outcome focussed care planning, aftercare support, and onward transition to other services as required.
  • Identifying risk, completing, reviewing, and following risk management plans where risk to lone work might be a factor for consideration. This can include risk of aggression, risk from peers, weapons, intoxication, and family crisis.
  • Planning of your own diary to manage caseload and meet the needs of young people and ensure that timeframes are adhered to for case records and report deadlines.
  • Ensure that all adult and children’s safeguarding issues are dealt with in line with Staffordshire’s safeguarding policies and organisation policies and procedures. This may involve supporting and educating family members, making safeguarding referrals to the local authority, and participating in all relevant multi-agency meetings.

Person specification

Qualifications

Essential criteria
  • Foundation Degree / HNC in related field which may include counselling, social work, nursing, psychology, human resources, social studies or equivalent experience

Experience

Essential criteria
  • Demonstrate experience and understanding of working with young people with complex needs
  • Experience of completing risk assessments
  • Excellent communication skills and a willingness to promote service user involvement
  • Case management experience & the ability to work under pressure
  • Experience of working within multidisciplinary teams with a proven ability to work collaboratively across professional boundaries
  • Experience of working with techniques such as CBT, Motivational interviewing, and Solution focused therapy

Skills and Knowledge

Essential criteria
  • Knowledge of drugs including alcohol and their effects
  • A commitment to and understanding of harm reduction approaches to working with young drug users
  • Ability to work with drug/alcohol users in a community environment
