Main Responsibilities
As a Care Co-ordinator you will be responsible for:
To undertake initial assessments within the area of specialism.To manage, monitor and review care plans involving users and carers and liaising with other workers and agencies as appropriate.
To be aware of the resources available within the specialist area, including other sources of funding so that the maximum benefit may be obtained for service users within budgetary resources of the Group.
To make use of the management information systems, including information technology, in the recording retrieval and analysis of information as required by the department.
Requirements:
Experience in a similar roleEnhanced DBS
