Your Key Responsibilities Optimising Medicines & Supporting the Practice Team Reduce GP workload by taking the lead on medicines-related queries, prescribing reviews, and medication adjustments. Drive improvements in prescribing safety and quality, ensuring compliance with best practices and regulatory guidelines.
Provide expert support to the wider practice team, offering guidance on complex prescribing decisions and medicines optimisation. Be a trusted point of contact for patients, ensuring they have access to safe, effective, and appropriate medication.
Lead on antibiotic stewardship and medicines safety, supporting practice-wide initiatives to enhance patient care. Clinical Duties Medication Reviews Conduct structured medication reviews, particularly for patients with polypharmacy, frailty, or multiple long-term conditions.
Chronic Disease Management Support patients with hypertension, Type 2 Diabetes, asthma, COPD, and other long-term conditions. Medicines Reconciliation Ensure safe, effective transitions of care by reviewing medications when patients move between providers.
Care Home & Frailty Reviews Work proactively to reduce inappropriate prescribing and optimise medication regimens for vulnerable patients. Dispensary Leadership & Development Oversee and develop the practice dispensary, ensuring it operates efficiently, safely, and in line with regulations.
Support and manage dispensary staff, providing mentorship, training, and ensuring compliance with audits and best practices. Drive quality improvement initiatives, identifying opportunities to enhance efficiency and patient experience in the dispensary.
