We have an exciting opportunity for an innovative and forward thinking dietitian to join our community based dietetic service to the proactive care team (PACT) based within our partner organisation Bradford District Care NHS Foundation Trust (BDCFT).

Main duties of the job

The Proactive Care Team help to reduce health inequalities and support a reduction in Accident and Emergency Department (AED) attendances and unplanned admissions whilst also improving access to other existing community-based services to help sustain good health outcomes.

PACT are a cross-organisational, culturally aware multi-disciplinary team (MDT) working in Bradford and Keighley to identify those with the highest need and deliver a blend of preventative and recovery focussed interventions in the community: to support a reduction of premature mortality and help deliver improvements for those who are frail and those close to the end of their life.

Detailed job description and main responsibilities

With the support of the band 7 dietitian team lead you will provide the dietetic service within this MDT and will help to develop the wider MDT to be skilled and knowledgeable in the early identification of key nutritional problems and good nutritional care.

Dual based at Horton Park, Bradford and Meridian House, Keighley, you will be required to work within patients’ homes and across a range of community locations in Bradford and Airedale

The post holder will be expected to participate fully in the training of ‘B’ and ‘C’ Placement student dietitians.

We are looking to recruit a dietitian with a broad dietetic knowledge base, excellent communication skills and the ability to manage a demanding caseload.

Applicants must be HCPC registered dietitians, who have obtained relevant clinical dietetic experience, and who are car drivers with access to a car.**

Experience

Essential criteria
  • Broad post-graduate dietetic experience in a range of clinical conditions.
  • Experience of working in a multi-disciplinary team and consultation with other professionals.
  • Experience of evaluating practice, service development and audit.
  • Experience of developing evidence-based resources and guidelines.
  • Dietetic teaching and training experience relevant to post.
Desirable criteria
  • Membership and experience of working with a relevant specialist dietetic/BDA groups.
  • Post-graduate dietetic experience working in community settings, particularly urban inner city areas.

Skills

Essential criteria
  • Excellent written skills with accuracy and attention to detail in written communication.
  • Able to utilise technology to support dietetic care including: Microsoft Word, Excel, PowerPoint, Teams, SystmOne or other electronic patient record.

Knowledge

Essential criteria
  • Understanding of Information Governance and Confidentiality.
  • Broad dietetic knowledge including in-depth knowledge of dietetic interventions for nutrition support.
  • Understanding of safeguarding in relation to working with adults.
  • Awareness of the operational aspects of the NHS.
  • Knowledge of the different dietary and social factors within Bradford District.
Desirable criteria
  • Understanding of health inequalities and the relevance to dietetic practice.

Qualifications

Essential criteria
  • BSc/MSc Degree in Nutrition and Dietetics/Post Graduate Diploma in Dietetics.
  • HCPC registered dietitian.
  • Clinical supervisory skills training.
Desirable criteria
  • Evidence of relevant recent continuing professional development training.
