The successful candidate will manage a case load of clients with issues around alcohol and substance use, delivering a range of psychosocial Interventions to help clients reach their health and wellbeing goals. Previous experience and/or knowledge of substance use services would be advantageous but is not essential as a comprehensive training package will be provided.

Similarly, previous experience within this field is desirable but not essential. Looking for transferrable skills such as caseload management, care planning, risk management and previous experience of working with vulnerable clients.

Key responsibilities:

Undertake effective case management and coordination with service users experiencing multiple needs in relation to homelessness, substance misuse and/ or domestic abuse.
Assess and engage with service users who are identified by/or referred to MBC Core Service and who ordinarily find it difficult to engage in specialist and /or statutory services or who find maintaining engagement to be challenging.

Complete comprehensive assessments, with service users identifying appropriate interventions, services, placements, and packages of support. Present funding requests or updates in relation to a service users’ outcomes to an appropriate panel and monitor and review agreed plan/placements/packages so service users can engage effectively and meet their individual needs and goals

Deliver a range of evidence-based interventions using a recovery and strengths based approach where solution focused intervention, motivational interviewing and communicating hope are central.
Identify, assess, and work therapeutically with risk and trauma. Practice will be creative and underpinned by knowledge of, and skills in evidence-based practice and psychosocial interventions.

Plan service delivery contact flexibly to meet individual needs and adopt a proactive approach to engagement so it maximises service accessibility. This includes delivering interventions peripatetically in a wide range of settings and communicating via a range of methods.

Work collaboratively with agencies, the service user and their personal support network to collate all relevant information to co-produce personalised person-centred & strength-based plans. Develop effective working relationships and communication with key voluntary and statutory services to embed a more solution-focused response to people with multiple needs.

Take the lead professional role where this is required and support colleagues from other agencies, including rapid response to crisis where applicable.
Assess risk in collaboration with service users, personal support network and other workers/agencies alongside using own professional judgement and team processes.

Provide assertive and persistent support to individuals and agencies to ensure effective engagement to achieve personal goals and aspirations.

Qualifications:

Maths and English GCSE or equivalent - desirable

Educated to degree level or equivalent - desirable

Access to own transport and a full UK driving licence. Business Insurance is required prior to starting if not already in place.

Knowledge:

Knowledge or experience of delivering creative and effective evidence based interventions to improve service user outcomes

A strong working knowledge of local services and ability to work collaboratively with partners with the ability to galvanize a team around a service user to achieve effective outcomes

Experience:

Proven experience of working with a range of vulnerable and complex service users in a range of settings
Experience working in crisis intervention, need and risk assessment, advocacy and support for vulnerable service users

Experience of using / inputting on to information and case load management systems

Skills & Abilities:

Effective time management and organisational skills and the ability to manage competing work demands

Ability to demonstrate good interpersonal and communication skills
