JOB DESCRIPTION Job Title: PCN Clinical Pharmacist Prescriber Team: Reporting to: Pharmacy Team Leader Responsible to: Business Manager for Townships 2 Primary Care Network Accountable to: Township 2 PCN member practices Location: Practices within the Primary Care Network Salary: Depending on experience Hours: 8 to 21 hours per week Contract Length: Permanent JOB SUMMARY The Clinical Pharmacist will work closely with the PCN Pharmacy Team and will provide clinical support to the pharmacy technicians and clinicians.

The pharmacist will support the PCN and practice clinical team with queries around medication, acute prescription requests and medicines reconciliation following discharge from another care provider, medication reviews, supporting self-care, minimising clinical risk, answering complex medicines queries, and aiming to reduce wasted medicines.
The post holder must be a prescriber. The post holder will be undertaking structured medication reviews, improving medicine optimisation and safety, supporting care homes, as well as running practice clinics, as set out in the national service specification structured medication reviews in the GP contract.
You will also be actioning acute prescription requests and conducting medicines reconciliation. KEY RESPONSIBILITIES The following are the core responsibilities of the PCN Clinical Pharmacist. There may be, on occasion, a requirement to carry out other tasks.

This will be dependent upon factors such as workload and staffing levels: To work as part of a multi-disciplinary team whilst in a supporting function to the Pharmacy Team and other clinicians and within agreed procedures Undertake medication reviews to support improved patient outcomes including: increased medication adherence stopping medications that are causing harm, Participate in pharmacy meetings, and other meetings to improve engagement and to promote issues relevant to prescribing and medicines optimisation.

Support the Medicines Optimisation team in the delivery of the Medicines Optimisation strategy in Sheffield. Handle function specific information, which may be sensitive, complex or confidential and appropriately recording, transferring and/or coordinating such information in accordance with the Data Protection Act; Caldecott Guidelines and the Confidentiality Code of Conduct Be responsible for the organisation, planning and of own workload to meet set deadlines.
Utilise GP practice clinical systems to conduct prescribing audits e.g. SystmOne/Emis. Ensure that all major MHRA drug alerts, where appropriate and within levels of competence, are acted upon within practices over an appropriate time frame and to advise on any medication changes that occur as a result.
Undertake training and development, to ensure the required knowledge and skills for practice-based work. Carry out other duties which are appropriate to the skills and competencies of the post holder and grade. Establish and maintain effective working relationships with GP practices, CCG and other healthcare professionals/stakeholders across the PCN.

Provide direct support and advice to GP practices and health professionals to promote and implement high quality prescribing to the benefit of patients. Provide peer support to other clinical pharmacists in the PCN
