Job Summary: To act as a focal point of communication between the GPs, pharmacist, receptionist, practice manager, patients and local chemist, as well as other members of the primary health care team, to ensure patients repeat medication is produced within the current protocols of the surgery.
Job responsibilities Work with the Practice Pharmacist to provide a Medicine Management Service and project a positive and friendly image to patients and other visitors, either in person or via the telephone To process, all repeat medication requests electronic and paper sent in by patients in accordance with Practice guidelines To process all repeat medication requests from the surgery NHS Mail box at least twice daily.
To collect and process repeat prescription requests from the fax machine, post box and reception. To process, all nomad medication requests electronically To liaise with patients and chemists regarding queries and requests. To ensure that patient queries on medication are highlighted to the relevant Pharmacist either Community or Practice.

Ensure adequate provision and security of prescription paper Initiating contact with and responding to prescribing requests from patients, other team members and associated healthcare agencies and providers To regularly go through prescriptions not picked up and unlikely to be picked up as a result of the timescale, to detail in the patient records and then destroy the prescription (2 months old) To adhere to the Death protocol in respect of prescriptions Be aware of monitoring requirements for selected drugs and follow up required monitoring with patients.

Ensure appropriate health checks and medications reviews are undertaken. Computer data entry/data allocation and collation; processing and recording information in accordance with Practice procedures. Establish and maintain filing and administrative systems so that written or computerised information is easily accessible and secure.
Employee responsibilities: Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way Maintaining security in accordance with Practice protocols Ensuring patient and carer records are up to date.
Dealing with confidential waste ensuring disposal into shredding facility provided Keeping working environment clean, tidy and free from hazard Alert other team members to issues of quality and risk Assess own performance and take accountability for own actions, either directly or under supervision.

Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance Work effectively with individuals in other agencies to meet patients needs Effectively manage own time, workload and resources Communicate effectively with other team members Communicate effectively with patients and carers Recognise peoples needs for alternative methods of communication and respond accordingly Apply practice policies, standards and guidance Discuss with other members of the team how the policies, standards and guidelines will affect own work Participate in audit where appropriate Competencies: Sound knowledge of EMIS Clinical System, Window and MS Office.

Awareness of data security requirements IT literate Good verbal and written communication skills Problem solving skills Flexible approach, team player, good sense of humour Knowledge of medical terminology. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data Personal/Professional Development: The post-holder will participate in any training programme implemented by the Practice as part of this employment, such training to include: Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work Health and Safety/Risk Management The post-holder must comply at all times with the services Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System The post-holder will comply with the Data Protection Act (1984) and the Access to Health Records Act (1990) Infection Control Applies infection control measures within the Practice to include: Universal hygiene precautions The collection and handling of laboratory specimens Segregation and disposal of waste materials Decontamination of instruments and clinical equipment Reporting and treatment of sharps injuries Dealing with blood and body fluid spillage Assist patients and colleagues in adopting sound infection control measures Equality and Diversity The post-holder must co-operate with all policies and procedures designed to ensure equality of employment.

Communication and Working Relationship The post-holder will establish and maintain effective communication pathways with colleagues, stakeholders, patients and their carers Recognise the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating Utilise communication skills to support patients to adhere to prescribed treatment regimens Anticipate barriers to communication and take action to improve communication when appropriate Rehabilitation of Offenders Act 1994 Because of the nature of the work, this post is exempt from the provisions of Section 4(2) of the Rehabilitation of Offenders Act 1994 (Exceptions) Order 1995.
