Take the lead role for management of level 2 and 3 critical care patients within ICU and ARCU. Co-ordinate and support ongoing rehabilitation of critical care patients on step down from critical care. Attend ICU MDT follow up clinic monthly alongside physiotherapy, medical, nursing and psychology colleagues.
Determine clinical diagnosis and develop effective treatment plans. Provide clinical leadership to other members of the Therapy team. Maintain own professional development and demonstrate high standards of clinical performance and professional practice.
Work within standards of practice according to the College of Occupational Therapy (COT) and local Trust policies. To act autonomously in relation to patient assessment and treatment with limited supervision To deputise for the Therapy Service Lead and Integrated team leader In-patients as required.
To undertake the training, supervision and performance management of Therapy staff and assistants allocated to the service. Be professionally and legally responsible and accountable for all aspects of own work, including the management of patients in your care.
Ensure a high standard of clinical care for the patients under your management. Accept sole clinical responsibility for a designated caseload of patients, and organise this effectively and efficiently with regard to clinical priorities and use of time.
Undertake the specialist and comprehensive assessment of patients referred from primary or secondary care, including those with a complex presentation, using investigative and using analytical skills Formulate individualised management and treatment plans, using clinical reasoning, and utilising a wide range of advanced treatment skills and options to formulate a specialised programme of care in conjunction with the patient and carers.
Plan a care programme for individual patients in conjunction with other professional staff within the multidisciplinary team (MDT). Provide specialist clinical expertise to colleagues within the team and referrers to the service. Communicate effectively with patients and carers to maximise rehabilitation potential and ensure understanding of condition.
Communication skills of persuasion, motivation and explanation to encourage patients to undertake their treatment programme. Use verbal and non-verbal communication skills to gain informed consent with patients where there will often be barriers to effective communication e.g. loss of hearing, altered perception, expressive or receptive dysphasia, pain, fear.
Assess capacity, gain valid informed consent and have the ability to work within a legal framework with patients who may lack capacity to consent to treatment. Communicate complex patient related information effectively to ensure collaborative working with MDT members and with MDT colleagues across health and social care sectors to ensure the delivery of a co-ordinated multidisciplinary service.
This will include case meetings and effective goal planning. Provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, to promote understanding of the aims of Occupational Therapy, and to ensure a consistent approach to patient care.
Train and supervise more junior staff and rehab assistants. Provide clinical education and training to Occupational Therapy students to graduate level on therapeutic skills and knowledge within clinical area. Maintain own clinical professional development (CPD) by keeping abreast of any new trends and developments, and incorporate them as necessary into your work.
Be an active member of the in-service training programme by the attendance and delivering of presentations and training sessions at staff meetings, tutorials, training sessions in house and by attending external courses and practising reflective practice.
Participate in the staff appraisal scheme and Personal Development Plan (PDP) as both appraiser and appraisee Undertake the measurement and evaluation of your work and current practices through the use of evidence based practice projects, audit and outcome measures, either individually or with team leader/manager.
