As the nurse specialist within the defined sphere of practice the post holder is responsible for providing specialist knowledge care to patients. They will act as the first point of contact who have need to access the specialist service. They will undertake a range of duties to ensure the effective delivery of care including undertaking specialist procedures, providing specialist advice, structured education and ongoing support to their client group, based on individual need.

Main duties of the job

To provide evidence-based care, assessment, advice and guidance on the management of diabetes for patients/carers within service level pathways and in line with local guidelines
