Job overview

The North Norwich Community Nursing team has an exciting opportunity for a registered nurse to join our friendly and supportive team of Nurses, Health Care Assistants, Assistant Practitioners and Nursing Associates, to provide nursing care and interventions across Norwich Place.

You will be based at Norwich Community Hospital working shifts 8am-6pm which include working weekends and bank holidays.

Applications are welcomed from experienced and newly qualified registered nurses. A supernumerary period is available and you will be supported with training and supervision. Please note due to operational demands, we are unable to support applications from students due to qualify in February 2026 or later for this post.

The Community Nursing service treats patients in their own homes, including residential homes and supported living complexes. We also provide some services in a clinic setting including our leg ulcer clinic and catheter clinic.

NCH&C is committed to the development of future health clinicians and there will be an expectation that alongside their clinical responsibilities the Band 5 Community Nurses support students, apprentices and junior colleagues in practice.

Main duties of the job

Key responsibilities include
  • To assess, prescribe and deliver care packages for patients. To work within the integrated team to facilitate early discharge from hospital.
  • To work within the integrated team to prevent unnecessary admission to hospital.
  • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
  • To process referrals though clinical assessment, evaluating the information received to prioritise a response and allocate to the appropriate service.
  • To liaise and communicate with the general public, other health care professionals, and multi-agencies on a daily basis.
  • To provide clinical assessment for incoming nursing referrals.

Detailed job description and main responsibilities

The workload requires a good range of clinical skills to be applied in managing our patients with varied health problems including diabetes management, traumatic and post-operative wound care, pressure ulcer prevention and management, catheter management, bowel care, and palliative and end of life care.

Many of our patients will have complex multiple pathologies and so we aim to provide holistic, patient centred assessments, and deliver individualised care.

  • Diploma or 1st Level Degree
  • Current NMC Registration
Desirable criteria
  • ENB 998 / Mentorship Preparation or equivalent
  • Leg Ulcer Management Course
