Key duties and responsibilities Support the role of the clinical pharmacists at Exeter City PCN in delivering medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings.
Support the role of the clinical pharmacists at Exeter City PCN in delivering medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings. Linking with local community pharmacists.
Medicines reconciliation post discharge and on other transfers of care Carry out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique). Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.

Day to day handling of prescription queries within area of own competence Delivery of training and development to practice based prescribing clerks and administrators to support improved co-ordination and effective pharmacy administration within General Practice Report on key performance indicators to demonstrate added value for patients and General Practice in accordance with NICE guidance Use and maintain information reporting systems and databases relevant to the position including incident reporting, medicines optimisation, prescribing data, cost savings, and patient outcomes Liaise with other pharmacy stakeholders e.g. Medicines Optimisation team, community pharmacy, and hospital pharmacy teams Handle function specific information, which may be sensitive, complex or confidential and appropriately recording, transferring and/or coordinating such information in accordance with the Data Protection Act; Caldicott Guidelines and the Confidentiality Code of Conduct Be responsible for the organisation and planning of own workload to meet set deadlines Utilise GP practice clinical systems to conduct clinical and prescribing audits e.g. EMIS Web, SystmOne Ensure that all major MHRA drug alerts, where appropriate and within levels of competence, are acted upon within practices over an appropriate time frame and to advise on any medication changes that occur as a result Carry out other duties which are appropriate to the skills and competencies of the post holder and grade of the post as the priorities of the service change Establish and maintain effective working relationships with GP practices, ICB staff and other healthcare professionals/stakeholders across the interface Promote cost-effective, safe, evidence based prescribing in accordance with national guidance and the local Medicines Optimisation Strategy Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients; General The post holder will develop a culture that promotes equality and values diversity The post holder must be aware of and committed to the Equality and Diversity policies of the appointing PCN and comply with all the requirements of these policies and actively promote Equality and Diversity issues relevant to the post Ensure the principles of openness, transparency and candour are observed and upheld in all working practices The post holder will have, or acquire through training provided by the organisation, the appropriate level of safeguarding and knowledge, skills and practice required for the post and be aware of and comply with the organisations safeguarding protection policies and procedures Ensure that any infection prevention and control issues are reported to the line manager/Infection Prevention and Control Quality: The post-holder will: Alert other team members to issues of quality and risk Assess own performance and take accountability for own actions, either directly or under supervision Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance Work effectively with individuals in other agencies to meet patients needs Effectively manage own time, workload and resources Participate in audit, where appropriate Personal & Professional Development: The post-holder will: Participate in the lead practices mandatory training programme and an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development Take responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work Health & Safety /Risk Management The post-holder must comply at all times with the PCNs Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the lead practices Incident Reporting System.

The post-holder will comply with the Data Protection Act (2018) and the Access to Health Records Act (1990). The post-holder must co-operate with all policies and procedures designed to ensure equality of employment. Co-workers, patients and visitors must be treated equally irrespective of gender, ethnic origin, age, disability, sexual orientation, religion etc.

Respect for Patient Confidentiality The post-holder should respect patient confidentiality at all times and not divulge patient information unless sanctioned by the requirements of the role. Communication & Working Relationships The post-holder will establish and maintain effective communication pathways with the following: Patients Clinicians, managers and practice and staff across the PCN Community Pharmacy Staff Hospital Pharmacy Staff and other Clinicians Care Home staff and residents Devon ICB Medicines Optimisation Team Special Working Conditions The post holder will be required to drive or be able to easily access all Practices The post holder may be required to undertake duties at any location in the community in order to meets service needs including aligned care homes Indirect exposure to emotional circumstances, i.e. terminally ill patients Job Description Agreement This job description is intended to provide an outline of the key tasks and responsibilities only.

There may be other duties required of the post-holder commensurate with the position. This description will be open to regular review and may be amended to take into account service development. All members of staff should be prepared to take on additional duties or relinquish existing duties in order to maintain the efficient running of the PCN.
