Job overview

The Nurse Practitioner within the Health and Wellbeing Team in HMP & YOI Feltham will be committed to providing healthcare services in custody, equivalent to those in the community. Post-holders will have broad experience of working in successful Child and Adolescent Mental Health Services and will have a good understanding of what is required to deliver an effective mental health service for young people in a custodial setting.

The post holder will participate in and co-delivering of training, attending team Reflective Practice as a key part of the role, facilitating a therapeutic culture and activities on the units including Community Meetings, contributing to Formulations and, where appropriate, providing supervision to prison staff.

The Health and Wellbeing Team will deliver a service that includes the following interventions: brief interventions, crisis resolution, early intervention in psychosis, dual diagnosis, learning disability and personality disorder. The Team will provide a multi-disciplinary approach, with a variety of therapeutic options, and an emphasis on (mental) health and wellbeing across the YOI.

Main duties of the job

To ensure duties are carried out in accordance with the service contract, the Health and Justice; Children and Young People Service Line, the YOI Service and the post-holder’s professional guidelines.

To be responsible for a caseload of complex assessment, treatment and consultation work with young people and adults, with mental health difficulties and learning disability.

To be responsible for initial assessment and developing, implementing and reviewing short term therapeutic interventions for referred young people where the work can be considered early intervention. The post-holder will carry out this process in consultation, as appropriate, with the young person and their family/carer, prison staff, fellow team members, other professionals, and the referrer.

To operate as a member of the multidisciplinary team providing a role that contributes to both the generic and specialist work of the Health and Wellbeing Team. Contribute to the integrated landing-based health support team for an identified landing, including being a visible presence on the landing and developing good working relationship with young people and staff.

Detailed job description and main responsibilities
  1. To work with all members of the team to uphold quality assurance systems that support performance against a) the contract, b) the Care Quality Commission’s Essential Standards of Quality and Safety, and c) Health and Justice Services KPIs.
  2. To use a range of outcome measures as required by the service.
  3. To be an active member of all clinical governances processes within the team.
  4. To attend formal prison-led meetings on security and safer custody (suicide prevention and violence reduction) and complex case review meetings (e.g. Child Protection, MAPPA, ACCT) when asked to do so by a senior member of the team.
  5. To contribute to policy reviews relevant to the Health and Wellbeing Team in HMYOI Aylesbury.
  6. To contribute to the production of reports, information and proposals that support effective delivery of care and an integrated approach to work with colleagues at local level or through external agencies.
  7. To assist the HWBT Service Manager in activities that ensure compliance with DH PHPQIs, Offender Care KPI’s and HMIP Expectations relevant to mental health.
  8. To take responsibility, under the direction of the HWBT Service Manager or Team leader, to achieve excellent performance for all relevant performance targets.

Person specification

PREVIOUS EXPERIENCE

Essential criteria
  • Previous post registration experience
  • Ability to keep accurate records.
  • Practical experience of providing high quality care and exceeding expectations.
  • Experience in providing and receiving supervision
  • Experience of running clinics and/or group work.
  • Experience of managing other staff
Desirable criteria
  • Experience of working within a secure environment / prison and/or working with adolescents
  • Experience of SystmOne IT system

EDUCATION AND QUALIFICATIONS

Essential criteria
  • Registered Mental Health Nurse/Registered General Nurse/Children’s Nurse or experienced CAMHS specialist Occupational Therapist HCPC registered
  • Evidence of personal and professional development
Desirable criteria
  • Post registration qualification appropriate to speciality
  • ENB 998 or equivalent

SKILLS, KNOWLEDGE, ABILITIES

Essential criteria
  • Knowledge of medication rounds and the safe storage and administration of drugs
  • Knowledge of, and ability to follow, NMC code of conduct
  • Knowledge of clinical audit process and an ability to use audit tools
  • Ability to assess plans, implement and evaluate patient care and act accordingly if untoward changes occurred
  • Knowledge of risk assessment and management and its implementation
  • Ability to teat pateinct with respect and dignity
  • Ability to use the computer to access patent information and use email
  • Able to use computer software packages such as MS word and Excel
  • Demonstrates the ability to manager work under pressure
Desirable criteria
  • Leadership skills (motivating, using initiative, change management)
  • Senior Nurse/ Bleep holding/ Bed management experience
