PCN Resource and Advocacy:Support the GP practices to refer patients to the Real Food Real Health ProgrammeDevelop and sustain excellent working relationships with the practice teams within the PCN, working in a way that is collaborative and informed.Promote social prescribing, its role in self-management, addressing health inequalities and the wider determinants of health.As part of the PCN multi-disciplinary team, build relationships with staff in GP practices within the local PCN.Be proactive in developing strong links with all local agencies to encourage referrals, recognising what they need to be confident in the service to make appropriate referrals.Plan, adapting and develop the service to deliver interventions which best meet the needs of the cohort you are supporting.Work with community services and assets to deliver a strengths-based, community-led approach.Maintain thorough and secure patient records.
Collating data and evidence to support research and evaluation of the service.Meet people on a one-to-one basis, making home visits where appropriate within organisations policies and procedures. Give people time to tell their stories and focus on what matters to me.Build trust and respect with the person, providing non-judgemental and non-discriminatory support, respecting diversity and lifestyle choices. Work from a strength-based approach focusing on a persons assets.Be a friendly and engaging source of information about health, wellbeing and prevention approaches.General admin tasks including: Managing spreadsheets used for triaging referrals and booking people on to various groups including troubleshooting Ensuring these are kept up-to-date with only relevant information stored on them Helping managing forms used for signing up to courses and receiving feedback including troubleshooting Pass feedback on to team and record appropriately for use in case studies and future planning and development Phone and email communication with patients throughout booking on and group running process, occasionally clinicians and other community partners and liaison.
