In Rotherham we have an exciting and rewarding opportunity to recruit a peripatetic Mental Health Clinical Practitioner, fixed term for 12 months to work across the 6 PCN's in Rotherham while been employed by RDaSH through the ARRS (Additional Roles Reimbursement Scheme).

We are seeking to find Mental Health Practitioner who can come and make a difference to patient care by providing high quality interventions in our new primary care integrated mental health service, you will be an important member of the primary care team, liaising with primary, secondary care and VCSE colleagues.

Applicants will work within the Rotherham PCN areas with requirements to travel between practices so the ability to travel across Rotherham would therefore be advantageous.

The practitioner will be embedded as part of the Primary Care Network Multi-Disciplinary Team and will be a part of the new Primary Community Mental Health Multi-Disciplinary Team with envisaged access to a wide range of support and expertise across the network patch.

It is envisaged that the focus of a practitioner’s work will generally be people whose needs can be met at in Primary Care support by a Mental Health Practitioner. If the persons needs can no longer be met within the primary care pathway, then the practitioner can facilitate onward access to other secondary care-based services, or third sector services integrated within the mental health pathway.

Main duties of the job
  • Based within PCN.
  • Provide a lead role in supporting Primary Care Networks in triaging mental health concerns and providing specialist advice.
  • Support GPs within the network to make appropriate referrals and support bridging a gap between primary care and secondary services while working in partnership across the system.
  • To act as first point of contact for GPs to gain advice and support regarding people’s mental health.
  • Where appropriate liaise with the GP team and the PCN regarding complex situations.
  • To be available to participate in primary care practice level meetings, including any meetings involving serious concern, i.e., safeguarding and risk management.
  • Be effective in multi-professional team working and service transformation.
  • To provide a comprehensive Primary Mental Health Nursing Service across the whole range of patients, service will be provided mainly via clinics and one-to-one interactions with service users which will include the provision of bio-psycho-social assessment and treatment delivery, including the role of Independent Non-medical Prescribing.
  • In order to work at this level, the nurse must be able to practice autonomously and be a confident decision maker.

Qualifications

Essential criteria
  • NMC Registered Nurse registered allied professional OT/Social work
  • A Degree-level registration qualification plus equivalent post-registration qualifications or experience to postgraduate level
  • The post holder will function as an Independent non-medical prescriber if suitable qualified as NMP and is a Nurse who is prescribing within their sphere of competence as defined by NMC
Desirable criteria
  • Registered Mental Health nurse
  • Teaching and Assessing Qualification.
  • Psycho-Social Interventions (PSI) Qualification
  • Non medical prescriber

Knowledge

Essential criteria
  • ln-depth knowledge in their specialist clinical field of: Mental Ill health conditions & diagnoses. Learning Disability (if applicable) & associated physical & mental ill health conditions & co / multiple morbidities. Substance Misuse (if applicable) to a care setting / service area. Appropriate evidence-based treatments & interventions for the clinical care setting / conditions/ service users you work with.
  • The principles of patient centred care & their application in practice. Mental Health Act ,Mental Capacity Act ,Safeguarding Adults & Children, Prevent, Information Governance, Human Rights ,Equality, Confidentiality.
Desirable criteria
  • Breadth of knowledge of Physical Health Assessment & Management

Is familiar with the use of relevant service user outcome measures. Demonstrates the ability to develop & educate other members of the multi-professional team to improve patient care & outcomes, leading by example.
  • Effective communication, leadership, improvement and change management skills. Effective oral and written communication skills. Effective interpersonal & non-verbal communication skills & awareness of personal impact. Competent in the use of IT / computer work-based information & care record systems.

  • Skills acquired in a breadth / diverse range of care settings / services organisations

Experience

Essential criteria
  • Working independently &/or effectively as part of a multi-professional team. Effectively managing inpatient care as a shift co-ordinator, Deputy Ward Manager or a Ward Manager &/or effectively managing a patient caseload in a community team setting. Capability to clinically lead & manage improvements in patient care across care pathways.
  • Confidence to discuss patient management / express difference of clinical opinion with other members of the Multi-professional team. Willingness to sensitively challenge & effectively address ineffective/weak/poor practice/systems where it/they occur. Being a strong clinical role model & leader, inspiring others to deliver safe, effective and personal care every time.
  • Engaging in effective Clinical Supervision, with the ability to reflect / critique / develop / & progress own practice. Providing Clinical Supervision to junior (preceptor) Registered Nurses/Student Nurses/Clinical Support Staff/ Nursing Associates/other Professionals.
  • Applying patient centred & recovery focussed approaches to the assessment & treatment of individual patients / service users, progressing over time to work across whole care pathways with increasing autonomy and a capacity for complex clinical decision making. Experience of engaging in service/quality improvement/evaluation work, research and or clinical audit.

  • Experience of working in a broad / diverse range of care settings / services / organisations.
