Job overview
To contribute to the assessment, implementation and delivery of high quality individualised patient care within a professional team. To demonstrate responsibility to patients, relatives and colleagues through contribution to the development and monitoring of agreed evidence based practice and to the health and social care programmes, under the supervision of a registered nurse/ care coordinator.
Main duties of the job- Undertake agreed activities as delegated by a registered nurse, with patients / service users under the direct / indirect supervision of the qualified staff.
- Provide care for patients’ physical and psychological needs, taking into account personal preferences and choices ensuring that a high standard of patient care is achieved.
- Assist the relevant professionals during assessment / treatment / intervention where required, carrying out therapeutic and recreational programs under the direction of a Registered Nurse.
- Carry out simple nursing procedures as instructed at the discretion of Registered Nurses, reporting on and recording findings, (e.g. blood pressure, temperature, pulse and respiration rate, maintaining fluid balance charts, diet charts, testing urine, weighing patients).
- Observe and provide feedback of current patient’s status and presentation to appropriate members of the multi-disciplinary team.
- Assist patients with activities of daily living that they cannot achieve alone, seeking advice from and reporting to Registered Nurses as required
- At the discretion of Registered Nurses, escort patients between departments in the Hospital and on therapeutic outings.

Experience
Essential criteria- Health and social care experience with direct patient involvement within the NHS/ Social Services, either bed based or community settings.
- Experience of basic nursing care
Knowledge
Essential criteria- Basic Maths and English skills
- Evidence of learning from reflection and self directed study
