Job overview

Reed ward, is a female organic assessment ward for older people presenting with psychological and behavioural symptoms of dementia. We are looking for someone to join our team who strives to provide person centred, compassionate care and is able to perform in pressurised environments.

You need to have the ability to have a non-judgmental attitude, be an active listener, and be perceptive to notice any changes in the service users mental and physical health. We are looking for someone who can be calm in difficult circumstances, and have an awareness of and the ability to adhere to strict confidentiality rules.

Work as part of a team and an individual practitioner, supporting service users to maintain their independence and attend to their activities of daily living. Applicants must be able to commit to time required for working on the wards (rotated shifts, shifts information below for your attention)

Main duties of the job
  • Supporting service users with their food and fluid needs and keeping accurate records of their intake
  • Supporting service users with their washing and dressing, and other hygiene needs
  • Completing therapeutic observations (1:1s) with service users, observing and monitoring risks and presentation
  • Lead activities on the ward that are dementia friendly
  • Keeping accurate notes/records of a service users presentation and clinical details
  • Supporting service users who are in states of distress and anxiety
  • Supporting service users who are presenting as a risk of harm to themselves and others
  • Supporting service users to maintain their independence
  • Work as part of an MDT to provide an accurate assessment of a service users physical and mental health
  • Completing physiological and neurological observations
  • Manual handling
  • Skin and wound assessment
  • Providing carer/family support of service users
  • Completing of mandatory training

Detailed job description and main responsibilities

You will be working closely with service users with varying degrees of cognitive impairments, and psychological and behavioural symptoms of dementia. You will be delivering a high standard of care, using excellent verbal and non verbal communication skills to engage with the service users, often when they are in periods of distress and acute confusion.
You will be supporting with their activities of daily living and maintaining their dignity at all times, supporting with tasks such as hygiene, and food and fluid intake. You will providing person centred care, delivering care that is shaped by that service users life history and likes/dislikes, looking at creative solutions to reduce distress and engage in activity.
Be able to do physiological and neurological observations and know how to and who to report any concerns found. Be able to support service users to reduce their risks such as falls, risk of harm to self and others, and skin damage. Complete skin assessments and monitor food and fluid intake.
Complete daily notes and other forms of documentation such as incident reports and NEWS forms. There will be the opportunity for development and learning new skills such as phlebotomy, doing ECG', supporting with monitoring of clinical equipment.

As part of the Team you will be expected to attend all mandatory training and keep up to date with your training records. As well as complete monthly line management supervision and yearly appraisals. You will also be supporting in the education/training of nursing, apprentice and medical students as part of MDT working.

Reed Ward is currently doing long days 07:00 - 19:30 and nights 19:00 - 07:30. The expectation of the role will be to work over a 24 hour rotation.

  • Previous experience within the care sector
Desirable criteria
  • Experience in Mental Health

  • Fitness to undergo personal safety training.

Qualifications

Essential criteria
  • Care Certificate – must be achieved within first 12 weeks of employment
  • NVQ L2/3 in H&SC or Diploma L2/3 in H&SC or apprenticeship L2/3 in H&SC

Skills

Essential criteria
  • Ability to work without direct supervision, liaising appropriately with other practitioners in the team
Desirable criteria
  • Experience of working with service users with dementia
  • Experience of working in mental health services
