Job overview

A new, exciting opportunity is being offered for enthusiastic and motivated senior trainees (ST4 equivalent or above) with an interest in paediatric emergency medicine (PEM) to join Gloucestershire Hospitals NHS FT. We are looking to expand our paediatric emergency area in the emergency department, and we are looking to appoint a senior fellow in paediatric emergency medicine for 2025/6.

The post will consist of 40% full time equivalent protected time for activities complementary to paediatric emergency medicine. The remaining time will be allocated to clinical Emergency Medicine work (60%) within the two Emergency Departments which includes time in the paediatric emergency area as well one twilight shift every fortnight on the Paediatric Assessment Unit (PAU).

The Senior PEM Fellow will be mentored by a senior educator within the department and will be supported by the department lead for paediatric emergency medicine as well as the nursing education and development team.

Main duties of the job

You will be allocated a Consultant Mentor and Educational Supervisor to support and guide your professional development during your time with us. We are very fortunate to be staffed by 19 experienced and enthusiastic Emergency Medicine Consultants, 10 Middle Grades (including one Associate Specialist), 6 ACCS doctors (St1 and St2) as well as a full complement of GPVTS, FY2 and FY1 doctors, ACPs and PAs.

Successful applicants will be welcomed into our vibrant and friendly multi-disciplinary team. There is a strong emphasis in team-work in our EDs and we work closely in collaboration with the Acute Care Unit, Ambulatory Emergency Care and Intensive Care.

Working for our organisation

The Emergency Departments within Gloucestershire Hospitals NHS Foundation Trust receive approximately 150,000 patients per year making us one of the largest units in the South West. Gloucestershire Royal ED is an accredited Level 2 Trauma Unit, receiving the most trauma cases of all Trauma Units in the region.
In addition, Cheltenham General Hospital (CGH) receives those patients suffering from acute stroke with an active stroke thrombolysis service. Within GRH ED there is a dedicated Paediatric Emergency area that receives ill and injured children.
Paediatric attendances constitute approximately 25% of all our attendances. There is Emergency Medicine Consultant supervision from 8am until midnight, seven days per week in GRH. CGH ED operates as an emergency department between 0800-2000 and as an MIIU between 2000-08000.

CGH does receive emergency patients, after being screened by the Emergency Medicine consultant in charge, by ambulance until 8pm each day, after which the ambulance patients attend GRH. There is Consultant supervision in the ED at CGH until 10pm each weekday and until 5pm at weekends (where a senior Emergency Medicine clinician is present until 10pm).

Detailed job description and main responsibilities

Clinically, as an ED senior doctor (ST4+ equivalent), your role will be in the assessment and treatment of the ill and injured patient as well as providing clinical advice to more junior ED clinicians, where appropriate. You will have the opportunity to be lead or part of the Trauma Team within the ED and to be the nominated Resus or Paediatric doctor of the day.

You will be expected to lead your colleagues in these areas as well as being an active team member. The clinical experience you gain will be diverse and interesting.

As an employee of the Trust the post-holder will work in close co-operation with and support other clinical, medical, professional, and managerial colleagues in providing high quality healthcare to the Trust’s patients.

Qualifications

Essential criteria
  • Full GMC Registration and a licence to practice
  • MBBS or equivalent medical qualification
  • Holds ETC/ATLS, APLS, ALS MRCEM
  • Must have approval of TPD to apply (if trainee)
  • Evidence of satisfactory / more than satisfactory progress through training, including annual review of competence progression (ARCP) outcomes.
  • Experience of running a general emergency department at night as the only senior decision maker – ie. ST4 equivalent or above.
  • Paediatric Emergency Medicine experience
Desirable criteria
  • Has instructor status in APLS or similar
  • Has other qualifications in paediatric medicine eg. DCH
  • ST4+ trainee in Emergency medicine or above

Experience

Essential criteria
  • Capacity to apply sound clinical knowledge, judgement & prioritise clinical need
  • Evidence of development of excellent diagnostic skills & judgement
  • Previous ACCS EM completion or other accredited route
  • Wants to develop one’s knowledge of paediatric emergency medicine
Desirable criteria
  • Postgraduate qualification in medical education, quality improvement etc
  • Quality Improvement experience
  • Teaching experience
  • Research experience

Knowledge and Skills

Essential criteria
  • Excellent interpersonal skills and ability to work in partnership with others across the range of professions and levels of seniority
  • Able to work independently and within a formal team structure
  • Competence in independent management of medical emergencies up to the level of an ST4 or equivalent
  • Able to manage simultaneous workstreams and deliver to deadlines
Desirable criteria
  • Previous experience of delivering simulation sessions and in debriefing
  • Knowledge of education/training programme evaluation methodology
  • Awareness of current educational initiatives with regard to patient safety, quality improvement, human error, risk management, crisis resource management, cross- speciality training

Leadership skills

Essential criteria
  • Evidence of effective team working and leadership, supported by multi-source feedback or workplace-based assessments
  • Self-awareness, with knowledge of personal strengths and weaknesses, impact and areas for development

Teaching

Essential criteria
  • Evidence of interest in and experience of teaching
  • Evidence of positive feedback from teaching
Desirable criteria
  • Evidence of previous simulation teaching experience
