This is an exciting post to increase and develop the provision of inpatient diabetes support and staff training into North Cumbria Integrated Care NHS Foundation Trust (NCIC). The post holder will be an integral member of the Diabetes Service specialist team, providing care, treatment and education to diabetes patients and their families.This will involve working closely with the community network of health care professionals to deliver an integrated package of care between hospital and primary care in particular in the support to empower patient to self-care with their insulin.

Main duties of the job
The Diabetes Service represents an integration of the entire multidisciplinary specialist team with responsibility for the quality of all diabetes services in Cumbria. We aim to improve the quality of diabetes care by delivering patient pathways with structured education programmes to support self-management, by supporting and training of primary care teams and providing excellent specialist diabetes clinical services in both in and out-patient settings.
A key aim of The Diabetes Service is to facilitate and encourage self-management by people with diabetes. Structured patient education programmes (DAFNE and DESMOND) are embedded in our pathways of care and training has been delivered to both primary care and specialist staff to undertake holistic care planning.
We provide speciality services including pregnancy, young person/ transition service and an insulin pump service. A Cumbria wide retinal screening service operates throughout Cumbria at a variety of locations.
This is a new role which will support patients to self-medicate and regulate their insulin following a stay in hospital or having been newly diagnosed.

Person specification
Experience and Knowledge
Essential criteria- Significant general nursing experience within the NHS
- To have detailed knowledge of all aspects of diabetes and associated risk factors.
- To have the ability to work autonomously, efficiently and professionally locality wide visiting patients in General Practice surgeries, Health centres, community and acute hospitals and emergency home visits
- Experience of teaching adults
- General IT skills including experience of windows based software and MS Office
- Understanding of current NHS issues and initiatives
- Knowledge of clinical governance and its application
- Up to date knowledge of the developments in diabetes care
- To have or be willing to become a DESMOND/ DAFNE/ Walking Away trainer
- Experience of undertaking clinical audit
Qualifications
Essential criteria- Degree level education
- NMC registration
- NST307 or willing to work towards
- Basic qualification/short course in specialist area (Diabetes e.g. Warwick Course A205, CIDC, Bradford Diabetes course)
- Adult Education Teaching Certificate
- Diabetes related qualification such as Warwick or Bradford
Skills and Apptitude
Essential criteria- Demonstrable clinical skills within specialist area, including physical assessment skills and basic life support
- Ability to obtain and evaluate information to aid decision making.
- Confident, diplomatic, good organisational ability, pleasant disposition, able to work in isolation but part of a team, co-operative & flexible, able to deal with difficult/emotional situations
Personal Circumstances
Essential criteria- Willing to undertake training as required.
