Paediatric Diabetes Specialist Nurse

PLEASE REFER TO THE ATTACHED JOB DESCRIPTION FOR FULL DUTIES OF THE ROLE. The successful candidate will be part of the South Tyneside and Sunderland Children and Young Peoples Diabetes Service and will be part of an MDT which operates from two base sites in Sunderland and South Tyneside.
The wider service includes a total of 6.1 WTE PDSNs comprising 5 Band 7 nurses, 4.3 WTE and 2 Band 6 nurses, including this post, 1.8 WTE and 1.0 WTE HCA delivering care to approximately 320 CYP with diabetes. The successful candidate will be able to operate from both the Sunderland and South Tyneside sites, depending on the needs of the service.

To coordinate the delivery and development of the specialist CYP diabetes service through the use of effective evidence based practice in line with agreed national standards for patient safety and monitoring To advise, support, train and supervise the HCA in the MDT To liaise effectively with the medical staff, currently 5 consultants, dietitians and psychology staff in the MDT To be accountable for the planning and delivery of safe, effective and compassionate patient care in relation to dedicated pathways of care within the specialism To continuously improve clinical care standards in line with the clinical governance framework To exercise accountability as set out in the NMC Code To ensure ongoing relevant continuing professional development in the field of paediatric diabetes in order to remain up to date in the rapidly evolving subject To ensure effective communication internally and externally regarding patient care and resources Provide education and training to clinical staff across the Trust Provide expert specialist clinical advice and support to patients and clinical staff within the Trust on the management and care of patients within the specialist area To provide direct access for patients, either face to face, video or telephone consultation To represent the service locally and nationally To play a key role in ensuring that national and local standards/targets for specialist area are met To manage or lead nurse led clinics in relation to reviewing patients and play a key role in optimising complex treatment regimes, facilitating hospital discharge and preventing avoidable hospital admissions To work with nurseries, schools and colleges to establish safe diabetes care for CYP when in education, using the regionally developed school care plan To undertake research relevant to the needs of the specialist area To undertake quality improvement projects as part of the MDTs QI programme To comply with the trusts vision, values and behavioural compact.
