Care Coordinator, KV Care, Bury

KV Care are a fast growing domiciliary care agency situated in Bury. Our services are expanding and we are looking for a fast-paced individual, with a strong passion for care, to join our team and support the deputy manager with our services in Bury and surrounding areas.
Job Description- To assist in the provision and development of a full and comprehensive domiciliary service to adults and children with disabilities and/or complex health needs or mental health needs. In accordance with the organisations assessment of need, giving the appropriate level of support to customers and their carers at all times, in line with the department procedures and policy statements.
- You will be expected to identify or create access to community facilities and social networks, leading to customers becoming less dependent on 'service support' systems and developing in there place a 'self-support' network of friends and associates within the person's neighbourhood.
- To be a first responder to new packages of care, assisting the deputy manager with the initial set up of care packages, consistently reviewing and feeding back to the deputy manager. To identify risks, concerns, good working practices and goals to aid the development of a full and comprehensive care plan and risk assessment.
- To undertake spot checks of the services you are responsible for overseeing on a rota basis alongside the deputy manager.
- Providing front line support to your customer base and staff teams
- Provide out of hours support and emergency response on a rota basis with the other managers.
- Assist the training manager to identify training needs within your care team
- Must be at least a Level 3 in Health and Social Care or degree level qualification in care/nursing with experience of providing and managing care services to vulnerable adults
- Full-time, Permanent
- Pay: £12.60 per hour
- 35 per week
- Minimum 2 years experience
