We are recruiting a Prescribing Pharmacist for a fully remote role supporting an innovative Hospital-at-Home service. This service enables patients to safely receive care and monitoring in their own homes through digital healthcare solutions, improving outcomes while reducing hospital admissions.
About the Role
As a Remote Prescribing Pharmacist, you will conduct telephone-based medication reviews with patients enrolled in the service. You will focus on optimising medication regimens, identifying issues early, and making prescribing decisions where clinically appropriate.
This is a temporary, ongoing position, with 40 hours available per week.
Key Responsibilities- Conducting structured medication reviews with patients over the phone
- Reviewing current medications for safety, effectiveness, and adherence
- Calling patients to discuss symptoms, side effects, and treatment outcomes
- Amending, initiating, or stopping medications in line with clinical guidelines
- Clearing and managing routine medication-related alerts
- Identifying red flags and escalating concerns to nurses or clinicians where required
- Documenting all consultations and changes accurately on clinical systems
- Providing clear advice, reassurance, and patient education
- Registered Pharmacist with Independent Prescribing qualification
- Experience conducting medication reviews (primary care, community, or remote settings)
- Strong clinical decision-making and risk assessment skills
- Confident telephone consultation manner
- Ability to work autonomously within a remote clinical environment
- Excellent documentation, organisation, and communication skills
- Comfortable using digital systems following training
- Fully remote role – work from home
