Do you enjoy working with people, dealing with a variety of challenges plus delivering actual benefits to make a true difference? This is an exciting opportunity for a motivated Pharmacist to join our Integrated Pharmacy workforce providing a service to a number of our PCNs within Wirral.

Each PCN is a group of GP practices working together to provide joined up services to support their patients to live happier, healthier lives. You will join an experienced and dynamic team that includes GPs, Nurses, Health & Wellbeing Coaches, Care Coordinators, Social Prescribers & Physiotherapists to provide clinical pharmacy services to our local population.

We have embraced collaboration to achieve a system wide overview of the patients’ medicines journey. This helps you identify how processes can be improved when patients move between care settings and become part of the system wide pharmacy family.

You will be supported to complete the CPPE pathway.

Main duties of the job

You will support these PCNs with the delivery of safe, effective systems for medicines optimisation, reducing waste and maximising patient outcomes through structured medication review.

You will contribute to new ways of working and using the skills that your role can bring to evolving PCN services.

You will be responsible for promoting good practice in line with clinical and operational guidelines and will clinically support the pharmacy technicians with medicines reconciliation post discharge and other medicines projects.

On a day-to-day basis you will spend time within the PCN hubs and in GP practices to answer clinical queries from patients, local GPs and clinicians, trusts and other services such as care homes. You will be supported to build strong relationships with community pharmacy and trust pharmacy colleagues.

Ultimately your role will be to support our patients to help them get the most out of their medicines and to play an active part in their own healthcare decisions.

Qualifications, Specific Experience & Training

Essential criteria
  • Vocational Master Degree in Pharmacy plus 1 year pre-registration experience
  • Postgraduate Pharmacy qualification or equivalent vocational experience
  • Post registration clinical experience in a relevant area of practice
  • Significant relevant and ongoing CPD
Desirable criteria
  • Independent Prescriber

Knowledge and Skills

Essential criteria
  • Understanding of the wider determinates of health, including social, economic and environmental factors and their impact on communities.
  • Good motivational skills and the ability to positively promote change in a complex healthcare environment
  • Excellent written communication skills
  • Produce timely and informative reports
  • High degree of computer literacy including database searching and manipulation of spreadsheets

  • An appreciation of the nature of General Practice and of Primary Care, including the concept of rational prescribing and strategies for improving prescribing.
