For more information on the Job Description and Responsibilities please click the link below. More detail about the role Our role can be challenging and is time sensitive in order to support patient flow through the acute hospital; but it is also rewarding and varied.
You will have the opportunity to work in many different departments and wards throughout the acute site, liaising with many different teams and other specialists in order to facilitate the appropriate discharge requirements for the patients that are referred to us.
You will also follow these patients up in the community, providing further assessments and supporting these patients to remain safely at home. We also require our support workers to assist with some basic administration tasks to support the daily running of the team.
This might include answering telephone and email enquiries, collating information and inputting referral information on our electronic health system. With this in mind some basic IT skills would be beneficial, however not essential as training can be provided to support these tasks.
We work closely with our community colleagues and have established ourselves as a link between acute and community care in order to provide seamless transition of care between hospital and home or other receiving destination. We are currently developing a Frailty pathway and are involved in many initiatives around length of stay and discharge planning and frequently receive excellent feedback from our patients as well as our professional colleagues.
We require a forward thinking, dynamic, motivated and creative individual to join us. Being able to work as part of a team and under pressure are essential skills that the successful candidate will hold. Experience from acute and community backgrounds would be preferred, but is not essential.

Please note that we cannot accept visa sponsorship for these 2 posts. Working Pattern: 37.5 hours per week. 7 days a week. We would consider full or part time applications. The team covers 7 days a week between the working hours of 8:00 to 20:00.
Flexible hours within these times could be considered for the appropriate candidate. Interview Date: To be confirmed. Pay:The salary advertised for this position reflects the Agenda for Change 2024-5 rates, inclusive of a 5.5% pay award implemented from October 2024.

If you join before this date, the pay award will be backdated to April 2024 or your start date, whichever is later.
