Job overview

The post holder will:

  • Lead and support the pharmacy Service provided to the Intravenous Outpatient Antimicrobial Therapy Service, which is part of the Virtual Ward /Hospital @Home service within the Integrated Care Division.
  • Work closely with the consultant microbiologists and other consultants, the lead and specialist nurses, and other staff members associated with the pharmacy service to the IVOPAT services.
  • Be responsible for coordinating pharmacy clinical services to the IVOPAT service, oversee and investigate medicines related safety incidents in the service, monitor financial and clinical governance in the service.
  • Lead on evidence-based prescribing advice to ensure effective, appropriate, sustainable, cost-effective and safe use of medicines and promote patient-centred care for all patients.
  • To lead on guidelines for the prescribing, administration and storage of IVOPAT medicines to facilitate good practice within the Virtual/Hospital@Home Services, within the Acute Trust and across the primary and secondary care interface.
  • To provide training and education to all levels of Healthcare professionals in acute and primary care whilst linking into national networks to learn about developments to share across the network.

Main duties of the job

Clinical Responsibilities:

  • Will be the senior pharmacist responsible for leading medicines optimisation to the IVOPAT Service for Wye Valley NHS Trust, providing pharmaceutical expertise to manage the implementation of national guidance and develop local guidance related to IVOPAT services.
  • Responsible for leading, developing and delivering highly specialist clinical pharmacy and medicines optimisation to the IVOPAT Service.
  • Use highly specialist knowledge of medicines to advise within a multi-disciplinary team of doctors, nurses, patients, carers and pharmacy staff on all pharmaceutical aspects of care. Receive and deliver highly complex, confidential and potentially contentious clinical information.
  • Responsible for the development, monitoring and review of Medicines Related Guidelines to the IVOPAT service.
  • Work with Microbiology and other specialty teams to deliver best practice care in line with national and local standards.
  • Support the ward based and dispensary clinical pharmacy service through clinical checking of prescriptions in line with professional, legal and best practice standards and acting as Responsible Pharmacist where appropriate

Education & Qualifications

Essential criteria
  • MPharm & registration with the General Pharmaceutical Council as a Pharmacist
  • Post graduate diploma in clinical pharmacy or equivalent experience to a similar level
  • Independent Prescribing Qualification or prepared to be working towards this
  • Evidence of CPD and study beyond registration in line with requirements for revalidation and demonstrating commitment to lifelong learning.
Desirable criteria
  • Membership of the Royal Pharmaceutical Society of Great Britain
  • Post Graduate qualifications relating to IVOPAT medicine or be willing to work towards this

Skills, Knowledge & Abilities

Essential criteria
  • Able to demonstrate a clear understanding of clinical pharmacy practice relating to Surgical Specialities
  • Able to process complex clinical information in an accurate and timely manner
  • Communicate effectively to all levels of staff using a variety of methods (written, verbal, presentation) with a wide range of people across the organisation.
  • Demonstrate a clear understanding of best practice and legal requirements relating to medicines use within the cardiology specialty

Experience

Essential criteria
  • Significant relevant, post registration experience as a hospital clinical pharmacist
  • Significant post registration experience in a hospital / primary care pharmacy setting with a proportion of time in a clinical role
  • Experience in motivating teams
