Join our Older Adult Community Mental Health Team (CMHT) at an exciting time of growth and development!We’re seeking highly motivated individuals, ideally with experience supporting older adults with a range of mental health needs, including complex emotional needs and trauma. Our CMHT provides secondary mental health care for people aged 65+, offering support for conditions such as psychosis, organic disorders, and emotional complexity.
We’re a forward-thinking, innovative team committed to service improvement and development. We offer strong clinical support, regular supervision, and truly value multidisciplinary teamwork. We’re proud of our welcoming and supportive environment, the last two agency nurses enjoyed working with us so much they joined us permanently!
We have robust appraisal systems and support ongoing professional development. We also regularly host students, who consistently give excellent feedback about their placements. Hours are Monday–Friday, 9–5 (some flexibility required), excluding bank holidays.
Due to the large North Lincolnshire area we cover, a full driving license and ability to travel efficiently are essential.

Main duties of the job
CLINICAL
S Undertake a comprehensive mental health nursing assessment and develop appropriate care plans.
S Carry out thorough risk assessment and implement appropriate interventions.
S At all times develop, implement and maintain high standards of nursing care. Maintain responsibility for managing a community case load, and coordinate care as appropriate within the community.
S To work within a multi-disciplinary team to ensure appropriate up-to-date interventions and liaison takes place to provide appropriate plans of care.
S To work within Trust and NMC policies and guidance on the administration and review of medication. To administer, supervise, monitor effectiveness of prescribed medication.

Qualifications
Essential criteria- R.M.N
- Evidence of post registration training
- Willingness to undertake ongoing training to meet CPD requirements
Experience
Essential criteria- Demonstrable post registration experience
- Working with the MHA and MCA
- Professional experience of working mental health
- Experience of MDT working
- Skills in coordinating and planning inter-agency approaches in mental health care
- Experience of assessing and managing risk
