Liverpool Early Help is seeking a passionate and dedicated Outreach Family Support Worker to join a well established team. Their approach is strength-based, focused on empowering children and families by addressing emerging concerns and working together to improve long-term outcomes.

Monday to Friday 9 to 5
35 Hours per week

Initial 3 month contract with the possibility of extension

Responsibilities:

Conduct early help assessments with families to identify needs and areas for support.
Develop and implement support plans tailored to the family's needs.
Chair Team Around the Family TAF meetings to review progress and ensure continued support.
Work directly with children and young people, gaining insights into their views and worries.
Collaborate with multi-agency partners to provide comprehensive support.
Use a whole-family approach to ensure holistic and effective interventions.

Ensure interventions positively impact children and young people's long-term outcomes.

Requirements:

Experience working with children, young people, and families, particularly in early help or social care settings.
Strong understanding of early help assessments, support planning, and multi-agency working.
Excellent communication and interpersonal skills to engage effectively with families, professionals, and young people.
Ability to chair meetings, review progress, and adapt plans to changing family needs.
