Clinical Pharmacist Prescriber

Job Summary:We are seeking an enthusiastic and skilled Clinical Pharmacist Prescriber to join our multidisciplinary team. The successful candidate will divide their time between face-to-face clinical reviews with patients and desk-based tasks to support practice needs such as prescription management, prescribing safety, clinical audits, and medicines reconciliation.
The role requires the post holder to hold an Independent Prescribing Qualification and to work collaboratively with a team of pharmacists in general practice, under the mentorship of a Lead Pharmacist.
As part of the team, you will take responsibility for managing areas of chronic disease and conducting clinical medication reviews, ensuring the proactive management of patients with complex polypharmacy. You will also provide support to practice staff regarding prescription and medication queries, helping to manage repeat prescriptions and address acute prescription requests.

Main Duties:
- Clinical Reviews: Manage a caseload of patients with common, minor, and self-limiting ailments, working within your scope of practice and limits of competence.
- Prescription Management: Signpost patients to community pharmacy and refer to GPs or other healthcare professionals as necessary. Handle acute prescription requests and ensure continuity of medicines supply to high-risk groups.
- Medicine Queries: Provide a telephone help line for patients with medicine-related queries, offer advice to GPs, practice staff, and other healthcare teams, and recommend appropriate solutions.
- Medicines Reconciliation: Reconcile medicines post-discharge from hospitals, intermediate care, or transfers to care homes, ensuring patients receive the correct medicines and identifying any discrepancies.
- Chronic Disease Management: Take responsibility for areas of chronic disease management and proactively manage patients with complex polypharmacy.
- Supporting Staff: Assist practice staff with prescription and medication queries, and support the repeat prescription system and systems for safer prescribing.
Qualifications:
- Essential:
- Registration with the General Pharmaceutical Council (GPhC)
- Masters in Pharmacy (MPharm) or equivalent
- Independent Prescribing Qualification
- Desirable:
- Membership of the Royal Pharmaceutical Society (RPS)
Experience:
- Essential:
- Previous experience demonstrating excellent attention to detail
- Experience of handling medication-related queries in a healthcare setting
- Desirable:
- Previous experience in general practice or within the NHS or healthcare sector
- Experience working in a confidential environment
