Palliative Care Clinical Nurse Specialist

We are looking for a highly motivated and dynamic nurse to join our community specialist palliative care team on a permanent basis. We are a friendly, professional and supportive multi-disciplinary team, who set out to provide high quality palliative care throughout Northamptonshire.Our aim is to empower others and deliver expert advice and support to improve the quality of life and death for people with advanced progressive illness. You will have excellent access to clinical and psychological supervision, relevant training, peer support and appraisal to underpin and develop you in your role.
Main duties of the job
The PCNS will join an integrated Specialist Palliative Care team that works in the community across three bases in Northamptonshire. They will be required to influence patient care by participating:
- High level of clinical practice
- Telephone triage of referrals into the Specialist Palliative Care (SPC) service
- Training and education
- Service development/leadership
- Research and audit
The PCNS will be aligned to a patient caseload, ensuring continuity of care in partnership with the integrated specialist palliative care team, GPs, acute trust partners and generalist community teams. There is an expectation to ensure care reflects the strategic priorities of NHFT; ensuring a flexible, responsive service that meets the local needs of people across Northamptonshire, whilst upholding Care Quality Commission requirements
This is a Band 7, part-time 22.5 hour permanent post you will have a nominated base at Cynthia Spencer Hospice; but there may be a requirement to work across the county.
Previous experience in palliative care and excellent communication skills are essential for this post. A non-medical prescribing qualification is essential.
Car owner / driver and current driving license essential or access to a car (unless you have a disability as defined by the Equality Act 2010).

Education and Qualifications
Essential criteria- Registered Nurse (Adult) /Current NMC Registration
- Recognised qualification in palliative care, or working towards
- Evidence of Continuous Professional Development
- Demonstrate commitment to and recent evidence of further education and personal development
- Completion of V300 Non-medical prescribing
- Completion of Advance clinical assessment skills
Knowledge and Experience
Essential criteria- Evidence of a high level of palliative care knowledge both cancer and non-cancer and its application to practice setting
- Significant level of recent Post registration experience/include experience of working in palliative setting
- Evidence of well-developed critical decision making skills which is evidence-based
- Sound knowledge and participation in research, measuring patient outcomes, audit
- Evidence of working with a wide range of people at all levels
- Demonstrates current knowledge of national and local policies and drivers
- Demonstrate a positive, flexible and adaptable attitude
- Forward thinking and proactive in decision making
- Led on audit/project management/service developements
- Demonstrates leadership in previously roles
- Recent community experience
Skills and Abilities
Essential criteria- Ability to holistically assess and monitor a caseload of complex palliative care patients
- Experience of advance care planning and supporting relatives/carers
- Ability to deliver advanced communication and advocacy skills
- Ability to promote a culture of patient-centred care, empathy and empowerment
- Able to respond flexibly to the changing needs of the service
- Ability to work autonomously, with initiative, recognising own limitations
- Experience of working within a pressurised environment
- Computer literate being able to write and maintain accurate documentation
- Experience of taking the clinical lead within a team with leadership responsibilities
- Experience of providing clinical advice and triage
- Experience of pharmacological interventions within the scope of palliative care
