Job overview

The Intensive Older Persons Community Team Manager role is responsible for the operational management of a range of aspects of service delivery within the team and for clinical/managerial supervision of professional staff according to the trust values.

To ensure safe caring, responsive, high quality service is provided to service users, their families and carers within the community by ensuring compliance with Statutory and Regulatory requirements and Trust Policies, pathways, standard operating procedures and protocols.

They will allocate work accordingly within the team to ensure effective and efficient use of resources to ensure service users and carers needs are met and monitor the team’s contribution and performance to the overall objectives of the Trust.

The Intensive Older Persons Community Team Manager will provide professional governance and leadership within their area.

Main duties of the job

SUPERVISORY RESPONSIBILITIES

The post holder will be responsible for Clinical/Management supervision to appropriate members of Trust staff. This should be carried out in line with Trust Policy

Detailed job description and main responsibilities

This position is within the Intensive Older Persons Service - a service which provides intensive support to older persons in the community so we can enable our older persons to receive required treatment at home rather than an admission to hospital, we also support our older persons in the community post discharge to ensure their care is seamless when transitioning from hospital to community.

This service operates from 8am to 9pm 7 days per week.

Qualifications

Essential criteria
  • Nursing degree in Mental health or equivalent, Dip SW or Dip OT or equivalent, or HCPC registered
Desirable criteria
  • Higher degree or post-graduate certificate in area of clinical / therapeutic practice

Experience

Essential criteria
  • Previous experience of working with Complex case management

Skills

Essential criteria
  • - Excellent communication skills.

  • Effective analytical and decision-making skills.

  • Competent and effective treatment skills.

  • An understanding of the policies and guidelines both local and national e.g.

National Institute for Clinical Excellence guidelines.
