Community Forensic Assertive Transition Service MH Nurse

We are looking for an experienced registered nurse (RLDN, RMN) with current registration with the nursing and midwifery Council (NMC). You will work as part of a multi disciplinary team, carrying a complex community caseload and providing in reach work to secure settings to support the transition of individuals leaving hospital settings that present a forensic risk.
You will have sound knowledge and understanding of working with individuals with a recognised Mental Illness/IDD and/or ASD , with a forensic risk. The importance of working with colleagues from multiple agencies to ensure a robust package of care is available.

You will develop and work in partnerships with Community Forensic Services, Adult Mental Health Services, Private Sector and Local Authorities. You will work directly with individuals and their families and carer groups. You will deliver person specific interventions inclusive of training packages You will have a sound knowledge of Mental health legislation including MHA, MCA including DoLs.
