Band 6 District Nursing Caseload Manager

  1. To have overall responsibility for the effective and efficient delivery of nursing care to patients within the clinical team. To work at all times within own competencies and the framework of the NMC Code of Conduct. 2. To lead a team of Registered Nurses and Health Care Assistants in the assessment, planning, implementation and evaluation of care for patients, in negotiation with the patient and supporting and involving carers as appropriate. 3. To prescribe safely, effectively and cost-effectively in line with Enfield Community Services policy and national guidelines. To ensure that prescribing competencies are maintained through a suitable reflective framework and with mentor support as needed. 4. To delegate appropriately to other team members. 5. To work with other caseload managers in the teaching, supervision and assessment of the ongoing clinical practice of team members and to be responsible for individual personal development review process. In conjunction with the Specialist District Nurse and Service Managers, identify the training needs of staff for the service training plan. 6. To be responsible for supporting/counselling staff for whom responsible and taking appropriate action, referring on as appropriate. 7. In conjunction with the Specialist District Nurse, organise regular meetings with the clinical team and provide opportunities for staff to participate in clinical supervision as provided by the Trust. 8. In conjunction with primary/secondary care practitioners, participate in identifying the health needs of patients to ensure the clinic resources are used appropriately. 9. To implement effective team working using a consultative approach, to achieve the aims and objectives of Enfield Community Services. 10. To ensure effective liaison with community nursing teams to organize agreed packages of care using a multi-disciplinary approach. To work closely with appropriate health and social care professionals. 11. To organise the overall daily management of nursing resources with regards to the needs of patients on the clinic caseload and taking heed of budget allocation and appointment times. 12. To liaise with district team manager or Specialist District Nurses to plan and co-ordinate annual leave, study leave. To participate in the management of sickness/absence and disciplinary procedures within the team. 13. To be aware of and support initiatives in District Nursing, which are linked to the Government agenda for the NHS. 14. To ensure that the dignity, religious and cultural beliefs of individual patients and staff are respected and that there is a positive attitude to equal opportunities
