Job overview

An exciting opportunity has arisen with STH NHS Foundation Trust for a physiotherapist who is keen to specialise in care and rehabilitation of the elderly.

As a specialist physiotherapist you will be working alongside a team of other professionals within an MDT to deliver rehabilitation and ongoing care. The team provides therapy and nursing input for 98 beds that are based within 3 care homes throughout the City.

As part of this role, you will be required to work flexibly across these units.

Our caseload is referred to us from the acute hospital wards for patients with complex conditions including fractures, frailty, cardiac and respiratory conditions, amputations and neurological conditions. Our patients are with us for a short-term rehabilitation programme to facilitate returning home safely and promptly.

You will need to demonstrate excellent interpersonal and communication skills. You must be able to prioritise tasks and manage time effectively, and be confident in your decision making. You must possess appropriate clinical skills and reasoning to deliver safe and effective physiotherapy treatment to our elderly client group.

This post will also provide opportunities to supervise and develop other staff members/students. You must be willing to show commitment to continuous development as we are a big advocate for staff development. For example, we have supported staff to complete courses such as PD Warrior, Bobath Module, Moving and Handling Key Trainers, etc.

Main duties of the job

To carry a variable clinical caseload of patients within the Intermediate Care and Rehabilitation Beds Service (ICRB).

To assess, diagnose, plan and develop specialist physiotherapy intervention programmes for patients with a wide variety of complex physical and mental health needs.

To participate and lead on the planning, delivery and evaluation of ICRB services within a designated area/unit, holding responsibility for defined projects.

To be an active member of the interdisciplinary Intermediate Care team in the provision of patient-centred care.

To seek and get support from the Physiotherapy Clinical Lead as needed.

To supervise/train junior physiotherapists/therapy assistants/students working within the service.

To develop and provide clinical expertise in a designated specialism and participate in the dissemination of evidence-based practice across the service.

Qualifications

Essential criteria
  • BSc or MSc in Physiotherapy.
  • Current HCPC Registration.
  • Evidence of Post-Graduate training relevant to Intermediate Care within the NHS.
  • Commitment to and documented evidence of Continuous Professional Development since registration.
Desirable criteria
  • Practice Educator Qualification.

Experience

Essential criteria
  • Significant experience in a range of clinical settings to include orthopaedics, neurology, general medical, respiratory.
  • Significant experience of working with older people in the above settings.
  • Demonstrates a clear understanding of Intermediate Care Services within the NHS.
  • Demonstrates a clear understanding of complex discharge planning in the NHS.
  • Team working and evidence of leadership experience considering NHS policy/procedure.
  • Demonstrates a good awareness of current National policies and priorities relating to Intermediate Care and Older People.
  • Supervision of junior physiotherapy/support staff and/or students.
Desirable criteria
  • To have worked in the Community and/or Intermediate Care services.
  • Experience progressing challenging conversations.

Special Skills/Aptitudes

Essential criteria
  • Extensive knowledge of musculoskeletal, neurology, orthopaedic and respiratory conditions within the elderly patient group and physiotherapy interventions to manage these.
  • Knowledge of Interdisciplinary team working within the NHS service.
  • To respond appropriately and efficiently to complex information/situations and maintain a professional approach under pressure.
  • To have good verbal and nonverbal communication skills using motivation, explanation and reassurance to actively engage patients and carers in their treatment programmes.
  • Knowledge and application of standardised NHS Intermediate Care assessments.
  • Ability to work autonomously and set own priorities.
  • Detailed knowledge of the principles of NHS clinical governance and its application.
  • Commitment to patient-centred, non-discriminatory practice.
  • To be able to maintain accurate written records & statistics using paper and Intermediate Care IT systems, European Computer Driving License or equivalent.
  • Knowledge of the principles of confidentiality and GDPR.
  • Evidence of performance with NHS PDR process and evaluation.
Desirable criteria
  • Presentation and training skills.
  • Ability to reflect critically and appraise own performance.
  • Understand and manage team dynamics within the remit of the role.

  • To be able to maintain clinical judgement under pressure.
  • The ability to understand and respect different points of view and work effectively with people from a variety of cultures.
Desirable criteria
  • To possess a full, clean UK driving licence and having daily access to a vehicle is essential.
