The Moor to Sea Community Nursing Team are based at three sites in the South of Devon. Ashburton/Buckfastleigh, Dartmouth and Totnes. Other than these small towns our footprint is largely rural.

We are looking for a full time Community Healthcare Support Worker (37.5hrs/PW) who is committed to high standards of care and who shows enthusiasm and a willingness to be a forceful advocate for the patients they look after.

We are focused on quality of care and offer a supportive network to allow any staff member to develop their potential through education and training alongside a friendly and supportive team.

Moor to Sea Community Nurse team covers Ashburton, Buckfastleigh, Dartmouth and Totnes and the surrounding areas. We are a busy, friendly and supportive team, committed to staff development through education and training.

Helping staff realise their potential in topics of interest to them and encouraging career pathways in Community Nursing is important to us. We are keenly aware of the pressures on nursing staff and how this can impact on the overall wellbeing of our staff and we will listen to reasonable adjustments to improve the working experience.

Main duties of the job

Venepuncture and sampling, basic and complex wound care, medication administration (with support). Safe and effective moving and handling and infection control practices. Good communication and documentation.

Detailed job description and main responsibilities
  • Undertakes a range of clinical care duties and implements planned programmes of care
  • To always work within own competence
  • To demonstrate clinical competence developed through continual professional development, reflective practice and maintain a skills portfolio relevant to the service specification
  • To undertake training to develop a range of knowledge and skills in order to deliver high quality clinical interventions
  • To recognise and appropriately address risk factors to clients and carers within their healthcare setting and feedback appropriately to the registered practitioner
  • To undertake designated nursing interventions recognising, recording and reporting all changes in the patient’s condition to the appropriate professional at the earliest opportunity
  • To report any untoward incidents, complaints and clinical emergencies to the appropriate professional within the appropriate timescale
  • To prevent adverse effects on health and wellbeing
  • To support good health for all patients within the local community
  • Can take instruction from the registered nurse accountable for each patient and undertake the individual nursing care needs presented by each patient under your care in the context of the prescribed medical treatment.
  • To comply with the uniform policy, maintaining a smart appearance at all times.
  • To ensure personal professional development and update.

Person specification

Qualifications and training

Essential criteria
  • NVQ Level 3 in Health & Social Care or equivalent qualification/experience
Desirable criteria
  • Care certificate
  • English & Mathematics at GSCE grade A-C (9-4) or level 2 functional skills

Knowledge and experience

Essential criteria
  • Previous experience in a care environment or able to demonstrate a caring nature/ life experience
  • Understands the need for strict confidentiality
  • Basic computer/keyboard skills
  • Good interpersonal and communication skills
  • Able to work under instruction, under pressure and as part of a team
  • Record keeping competency in handwritten and electronic records
  • Able to demonstrate empathy, sensitivity, and to adapt communication style to circumstances
  • Able to manage stress in themselves and others
  • Able to offer support at times of emotional distress
  • Willingness to undertake new skills and training
  • Able to work as a team member or independently
  • Ability to adapt behaviour to changing and challenging situations
  • Flexible, adaptable, dependable, and punctual
  • The post holder must demonstrate a positive commitment to uphold diversity and equality policies approved by our Trust
Desirable criteria
  • Working knowledge of complex care needs of some individuals/families including Safeguarding Children & Adults, Domestic Abuse and Mental Health
  • Healthcare competencies appropriate to area of work
