Job overview

Gloucester ICT Community Nursing Team are looking for an enthusiastic and dynamic individual to support the team in delivery of patient care in their own homes.

Community experience is essential with the ability to demonstrate a full range of community competencies. You will support and lead a team whilst caring for a complex and diverse caseload, working closely with your peers, wider integrated community team and GP practice staff.

The role will entail triage in the referral centre (usually 2-3 times a month and a late shift (usually 1 a week) .

Our shifts are 8-4pm, 12-8pm- with one weekend a month on a rolling rota (days in lieu given).

Main duties of the job
  • To enable patients to live well within their communities, with a focus on physical health needs and wider holistic needs.
  • Lead Multidisciplinary Team meetings meeting as required- including GP supported clinical meetings (Gold Standards Framework or Case Management reviews)
  • Communicate and liaise with the Multidisciplinary ICT, Gloucestershire Care Services (GCS) and Gloucestershire County Council, Gloucestershire Hospitals NHS Foundation Trust as well as other referring acute and community hospitals
  • Discuss patient's conditions with them as appropriate and relay accurately and contemporaneously back to appropriate colleagues.

Detailed job description and main responsibilities
  • To Practice as a senior member of the community nursing team, working within the Integrated Community Teams (ICTs)
  • To provide a case management approach to care for an identified number of patients working closely with GPs, the ICTs and other Statutory and Voluntary agencies.
  • To provide advice, information and education to patients and carers - enabling patients to enjoy maximum independence, health and wellbeing, enabling people to live well at home
  • To complement the urgent care services to support admission avoidance .
  • To support peers and operational managers.

Person specification

Length and nature of appropriate experience

Essential criteria
  • Experience of working without direct supervision
  • Experience of managing long term condition and complex patients
  • Evidence of post-registration education and training
  • Consolidated experience of community nursing practice
Desirable criteria
  • Demonstrate working in a team
  • Managing and leading other members of your team
  • Demonstrate understanding of triage process.

Appropriate experience

Essential criteria
  • Community Nursing Qualifications-For the DN role, the Specialist Practitioner qualification in Community/ District Nursing is essential at diploma or degree level If the DN SPQ is not held, the post holder must hold a degree in a health related subject or be working towards this with over 50% of level 3 or 6 credit already obtained.
  • A teaching and assessment or equivalent mentoring and assessing qualification is essential and ability to demonstrate it's utilisation.
