Job overview

We have a great opportunity for a full-time band 4 Nurse Associate to work in the Dementia & Older People’s Mental Health Team based in Worthing West Sussex.

The Nurse Associate role is pivotal to the review process of older people and their carers who require interventions from our services. This will involve direct working with people in the community and home settings, liaison with primary care services and other providers of support within the local community.

We will take a keen interest in helping you to develop the skills important to this role and the post-holder will be encouraged to develop special interests and expertise in line with service and team objectives.

You will be part of a highly skilled and supportive multi-disciplinary team. You will receive regular managerial and clinical supervision. This is a challenging but rewarding role as we receive positive feedback from our service users and carers.

The post holder will be expected to travel across the geographical area (Adur, Arun & Worthing) covered by the team, the ability to travel to remote areas is essential.

Main duties of the job

The role of this post is to actively engage those requiring an increased level of support with the aim of keeping people within their home and maintaining their wellbeing and independence as far as possible. We are looking for individuals who are dynamic, compassionate, good communicator who are able to demonstrate a holistic and person-centred approach to their work.

You will also need to be able to work cohesively with other services both within our Trust, and with other partner agencies. We follow a multi-disciplinary approach and within that context you will manage a caseload of older adults.

Detailed job description and main responsibilities

Clinical
  • To contribute to the management of a team caseload of clients with complex needs, using CPA principles, evidence based/client centred principles to assess, plan, implement and evaluate interventions, which will be delivered in the Dementia and Older Adults Mental Health Service.
  • To hold a caseload as Lead practitioner and attend relevant MDT meetings
  • To regularly support duty
  • To assist with specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the care pathway
  • To assist and provide risk screening and assessment as well as risk management planning
  • To contribute to and facilitate evidence-based assessments and individual and group interventions
  • To contribute to the maintenance and development of the Service
  • To regularly support with supervising students on practice placement, and junior staff as appropriate .

Knowledge/Experience

Essential
  • Post qualifying experience in the relevant care group
  • Experience of working alongside people with complex health problems in the designated care group
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
  • Experience of working consultatively with professionals
  • Experience of conducting clinical assessments including risk assessments
  • Experience of partnership working within and across various statutory and non-statutory teams and agencies
  • Experience of working within the CPA process
  • Understanding of clinical governance
  • Previous experience of working in the community

Qualifications

Essential
  • Registered Associate Nurse with current registration appropriate to the job role
  • Completion of Mentorship Course/ENB equivalent
  • Evidence of continuous professional development
