Job overview
An exciting opportunity has arisen for a full time enthusiastic, health care assistant to work in our community branch of the Mental Health Rehabilitation Service, We are based on Bodmin Hospital site in Cornwall.
The Community Mental Health Rehabilitation team provides a service across the whole county of Cornwall supporting people recovering from complex and enduring mental health issues. The community Rehabilitation Team operates alongside the inpatient ward Fettle House allowing the team to follow through a service offering a specific pathway for the patients as they resume/ establish independent lives outside of the hospital setting.
We take a person-centred approach in developing individualised dialogue plus care plans. These care plans set out key therapeutic interventions that are important and meaningful to the individual. For example, this will include building confidence in managing their day-to-day task such as managing finances, planning and cooking meals, developing a daily routine, linking the person in with local services.You will be responsible for reporting back to the MDT. You will also be responsible for supporting with the continued review and development of the rehabilitation service. Regular individual support and supervision is provided. This is a full-time post working a shift pattern, 08:30-2030 working 7 days per week.
Main duties of the job
This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
- Monitor health needs through observations and report changes to care coordinator so that the care plan and/or risk plan can be kept fully up to date.
- Provide housing support where required.
- Support the creation of Crisis and Contingency Plans.
- Undertake Carers Assessments and development of Carers Support Plans.
- Monitor basic health e.g. diet, hygiene and sleep and assist in addressing identified needs. Work with service users and their carers to achieve the objectives of the care plan.
- Encourage social/physical activities as appropriate to the service users’ needs.

Person specification
Education
Essential criteria- NVQ3 in healthcare or equivalent qualification or experience.
- Training to level 2 in Safeguarding children and young people: roles and competencies for Health Care staff
Experience
Essential criteria- Working with clients with a range of mental health problems.
- Experience of working in a multi-disciplinary and multi-agency environment.
- Working within a community setting in either the statutory or voluntary sector.
